How Much Does Semaglutide Cost Without Insurance? $254 a Month at 126 Clinics

What you actually pay for semaglutide when insurance is out of the picture: the published prices of 126 clinics that name it, the makers' own cash prices for Wegovy and Ozempic, and the questions that keep a $199 price from turning into $400.

The short answer

If you pay for semaglutide yourself at a local clinic, plan on about $254 a month. That is the middle of what 126 clinics across the country publish on their own websites for a semaglutide program. Half start at $254 or less. A quarter start at $199 or less. A quarter start at $350 or more.

The spread is wide: thirteen clinics start under $150, ten start at $500 or more, and the highest published figure is $1,500 a month. The clinic is rarely the reason for that gap. The medicine is, and so is whether the visits are in the price.

These are the clinics' own numbers. We pulled them from the directory on October 29, 2026, and counted a clinic only when its own price words name semaglutide. Each one is listed with the page it came from and the date we read it on semaglutide and tirzepatide prices by state.

Three different prices for the same word

Compounded semaglutide at a clinic. A compounding pharmacy makes it to the clinic's prescription, usually a vial you inject weekly. This is what most cash-pay clinics sell. The 14 clinics that say "compounded" right in their price start at a median of $250 a month. The lowest published starting price in the directory is $63 a month, in Kalispell, Montana; several others start at $79 to $99.

Brand-name Wegovy or Ozempic through a clinic. Six clinics in the directory name Wegovy or Ozempic in their price. They start at a median of about $315 a month; one med spa prices branded semaglutide at $500 to $1,000.

Brand-name pens or pills straight from the maker. Novo Nordisk sells to self-pay patients directly. We keep each figure with its source and the date we read it on the Wegovy price page and the Ozempic price page. As of our last read:

  • Wegovy pens are $349 a month at every dose up to 2.4 mg, and $399 for the 7.2 mg pen. New self-pay patients get the two lowest doses for $199 a month for the first two fills, an offer the maker dates through December 31, 2026.
  • Wegovy pills are $149 a month for 1.5 mg, $199 for 4 mg, and $299 for 9 mg and 25 mg.
  • Ozempic pens are $349 a month for the 0.25, 0.5 and 1 mg doses and $499 for 2 mg, with the same $199 offer on the lowest doses. Ozempic is approved for type 2 diabetes, and this figure comes from the maker's announcement rather than its own price page, so confirm it there before you buy.

Self-pay prices from the maker do not count toward a deductible, and the maker can change or end them.

Notice that the maker's $349 pen sits at the upper quarter of what clinics charge for compounded semaglutide. If a clinic quotes $350 or more for the compounded version, ask why the brand-name one would not cost the same or less.

Does the price include the medicine?

Fifty of the 126 clinics say plainly that the medication is in their monthly price. Among those, half start at $299 a month or less, a quarter at $219 or less, and a quarter at $400 or more. That $299 is a real all-in number most months.

None of the 126 says outright that the medicine is extra, which only means the page does not say. A "program from $125 a month" that never mentions the medicine is a price for visits, with the semaglutide billed on top. So ask each clinic the same sentence: *does that include the semaglutide, every month, at every dose?*

The dose goes up, and so does the bill

Semaglutide starts low and is raised over several months. Many clinics price it by dose, in their own words: one Oklahoma clinic starts at $280 a month and goes "to $400/month at the 2.4 mg max dose"; one South Dakota clinic charges "$199, $299, or $399 based on prescribed dose"; one Colorado spa's $99 "is for your first month of Semaglutide only." Others say "any dose $299/month" or "$249 per month, no matter the dose." Both are honest; they are different deals.

Sixty-two of the 126 clinics publish a range rather than one figure, and the median top of those ranges is $300 a month. If a clinic shows you only a starting price, ask what month four costs.

The first visit

Most clinics charge for the first appointment: a history, usually bloodwork, and the prescription. Across all weight loss clinics publishing a first-visit fee the median is $140, and 213 say the first visit is free. Among these 126 semaglutide clinics, 35 publish a fee, median $125. A free first visit means the clinic earns on the monthly program, so make sure that monthly price is on the website before you book. What a first visit costs lists every published fee by state.

By state

Only two states have ten or more clinics publishing a semaglutide price, so this table is short on purpose.

StateClinics publishing a semaglutide priceHalf start at or under
Florida21$275
Texas12$250

California (9 clinics), Utah (7), Arizona (6), Colorado (6) and Massachusetts (6) come next. Everywhere else the count is too small to call typical, so look at your own state's list. Within one state the cheapest and dearest clinic are often $400 apart, a wider gap than between states.

Online versus a local clinic

The online providers listed on this site charge $99 to $548 a month, most with the medicine included at the lower end. That beats the median local clinic, though not the cheapest ones. What the local clinic gives you is a person in the room: bloodwork, an exam, someone to call when a side effect turns up. If you have a thyroid history, pancreatitis, gallbladder trouble or a long medication list, that is worth paying for. Otherwise, start with the maker's pill or an online provider. Online vs. local GLP-1 provider walks through the trade-off.

What about insurance?

Insurance sometimes covers Wegovy for a diagnosis of obesity, usually after prior authorization, and Ozempic for type 2 diabetes. It almost never covers compounded semaglutide. If you have coverage worth trying, start with your own doctor and pharmacy; if not, every figure above is the real price.

Five questions to ask before you pay

  1. Is this compounded semaglutide or brand-name Wegovy or Ozempic, and what does each cost?
  2. Does the monthly price include the medicine, at every dose, every month?
  3. What does the price become at the higher doses, around month three and four?
  4. Are the first visit and the bloodwork in the price or extra?
  5. Can I stop month to month, or am I signing up for a term?

A clinic that answers all five on its website is a clinic you can compare. One that will only tell you on the phone wants you in the chair first.

The bottom line

Without insurance, budget about $254 a month for compounded semaglutide at a local clinic, or $299 if you want the medicine in the price. Check the maker's own self-pay prices first: $149 to $299 a month for the Wegovy pill and $349 for the pen, with a dated $199 offer on the starting doses. The cheapest local clinics beat all of those; the most expensive charge $500 and up and should be able to say exactly what that buys.
