What is Smiley Aesthetics?

Smiley Aesthetics is a medical spa group with seven locations, in Nashville, Gallatin, Soddy-Daisy, Collierville, Knoxville, Bowling Green and Rome, across Tennessee, Kentucky and Georgia. Its main business is injectables and skin treatments, including Botox, Letybo, dermal fillers, Sculptra, Scarlet SRF microneedling, facials, IPL and laser hair removal. Medical weight loss with GLP-1 medication is one of its services, alongside bioidentical hormone replacement therapy and CoolSculpting Elite.

The weight-loss program is titled physician-supervised and invites bookings with its APRNs. No prescriber is named for that program. The medications it names are liraglutide, semaglutide and tirzepatide. In its own words: "We keep it simple by offering only high-quality Vitamin Injections and GLP-1s." What does it cost?

The GLP-1 subscription runs $299 to $575 per month, and that price includes the medication. The initial consultation is a separate $150 fee, which covers lab ordering, EMR setup and confirming whether you are a candidate. The subscription is billed monthly with no required commitment. Fat burner shots are $39 each.

Costs may change after the first month, depending on dosing and nutrition needs. Its online shop also lists compounded semaglutide at $169 to $229 a month and compounded liraglutide at $315, which do not match the subscription range. Ask which price applies before you book. It does not say whether insurance is accepted, and it does not mention HSA or financing.

What should you ask before you book?

Ask who signs the prescription and whether that person is a physician or an APRN. Ask whether the monthly fee covers the medication, labs, injection supplies and check-ins, since its program list says it does. The medication is compounded through Strive Pharmacy, a licensed compounding pharmacy in Tampa, Florida. In its own words: "compounded medications are not the same as commercially manufactured, brand-name pharmaceuticals." Ask whether a brand-name option exists and what the difference means for the product you would receive.

Ask what follow-up visits and lab work cost after the first month, and what happens if your primary care labs are older than 6 months. Ask whether the $150 fee is refunded if you are not a candidate. Ask how to cancel, and read the cancellation and refund policy before you pay.

The weight-loss program can be done virtually. Its booking options list Tennessee, Georgia, Ohio, Missouri, Kentucky and Kansas, while Arizona and Colorado appear only in its service area list, so ask whether you qualify. Who is it a good fit for?

This is a good fit if you want aesthetic treatments and a mostly virtual GLP-1 program from the same practice, and you are comfortable getting answers about the prescriber, the pharmacy and the monthly price before you pay.
