What is Simply Revived Aesthetics?
Simply Revived Aesthetics is a medical spa at 151 E Wisconsin Ave. Suite 1, Pewaukee, WI 53072. That covers an initial evaluation with a weigh-in, dietary education and a demonstration of self-injection, three 30-minute side effect visits, and up to six further check-ins. It does not say whether medication is included in that price. Labs are separate. If you have no labs from another provider in the past 6 months, you will be sent to an off-site lab, usually at a cost of less than $40 to you.

Ask whether the $1125 includes the medication. If it does not, ask the monthly cost, and whether the medication is brand-name or compounded. Ask what the program costs after the first three months, and what the labs cost in total, including the labs due after six months of treatment. Ask whether insurance is accepted and whether the consultation has a fee. Ask how side effects are handled between scheduled visits and who to call. If you join a membership, ask how cancellation works, since the 12-month commitment is the only term stated.
Who is it a good fit for?
This is a good fit if you want nurse practitioner visits and a fixed three-month fee for a weight loss program, and you also want aesthetic or IV services at the same clinic. If you need the medication cost in writing or a clear answer on who prescribes, get those answers by phone before you book.
