What is ReforMedicine?
ReforMedicine, pronounced "Reform Medicine," is a direct pay medical clinic in Western Wisconsin. Its main work is family medicine and primary care for patients of all ages. It also offers a Medical Weight Loss Program, low cost lab tests, an at-home sleep study, and on-site or near-site clinic services for employers. It has offices in Eau Claire, Menomonie, Hudson, Lake Hallie and Baldwin. Its Google listing shows a rating of 5 from 2 reviews.
The physicians on the prescriber list are David J. Usher, M.D. and Michael Braunsky, D.O., DO. The others are nurse practitioners and physician assistants, including Renee Carlson, ANP-C, APRN, DNP, FNP-C, Dana Butterbrodt, APRN, DNP, FNP-C, MBA, and Ellen Canopy, PA-C. Usher, Carlson, Butterbrodt and Canopy each carry the title Medical Weight Loss Specialist.
The weight loss program is open to adults 18 and older who are at risk for or suffering from obesity. Before you enroll, you must watch an introduction video. In its own words: "Patients with chronic conditions are welcome and the medical provider will monitor your conditions and medications as part of this program." The program does not routinely involve surgery.
This listing is for its Hudson office at 1101 Carmichael Rd Ste G601.
What does it cost?
Enrollment is a $999 bundle, due in full when you register. It covers the start-up visits and labs: a welcome appointment with a medical assistant, an EKG, a body composition analysis, a provider appointment, and at least two health coaching appointments. Follow-up labs at 3 and 6 months and at your goal weight are included as well.
After enrollment you pay per visit. Monthly provider follow-up visits cost $148 each, and health coaching visits cost $60 each after the ones included in the enrollment fee. Prescription weight loss medication is optional, and its cost varies by prescription. It does not say whether any medication is included in the $999. In its own words: "ReforMedicine Medical Weight Loss is not a contract for care." Re-start fees are covered in its Medical Weight Loss Policies, which you should read before you pay.
It does not publish a consultation fee, prices for its low cost lab tests, or whether it accepts insurance.
What should you ask before you book?
Ask whether the $999 includes any weight loss prescription, and what that prescription would cost each month. Ask whether the drug is brand-name or compounded, and who writes the prescription and adjusts it at follow-up.
Ask what a single lab test costs outside the enrollment milestones, and what a consultation costs beyond the enrollment bundle. Ask whether any visits can be done by video, since that is not stated. Ask which office you would use and what its hours are, since office hours are not listed.
Ask whether the clinic bills insurance at all. Ask how you would stop the program and what a re-start fee would be. Get both answers in writing before you pay the $999.
Who is it a good fit for?
This is a good fit if you are an adult who wants a medically supervised weight loss program, can pay $999 up front, and is comfortable paying per visit after that. If you need insurance billing, a fixed medication price, or a video visit option, get those answers before you enroll.
