What is ProVita Wellness?
ProVita Wellness is a functional medicine clinic at 18650 W Corporate Dr, Suite 126, in Brookfield, Wisconsin. Functional medicine is its main work, an approach that looks for root causes rather than only symptoms. Visits can be in person or virtual. Crystal Dillett, APNP, is the founder and a Certified Family Nurse Practitioner, and she has over 12 years of experience in conventional medicine caring for patients with chronic illness. The other prescribers are Annalyse Victor, APNP, an Integrative Oncology Nurse Practitioner and Certified Family Nurse Practitioner; Jenna Byom, APNP, a Certified Family Nurse Practitioner and Certified Integrative Nurse Coach; Crystal Brandt-Holborn, APNP, a Certified Family Nurse Practitioner and Certified Functional and Nutritional Medicine Provider; and Tracy Kersten, APNP, a Certified Family Nurse Practitioner and Registered Yoga Teacher.
The service list is long, running from Functional Medicine and Mold and Lyme to Hyperbaric and Red Light Therapy. For weight, hormone and peptide care, the relevant entries are Weight Management, Hormone Therapy and IV & Peptides. Its Google listing shows a rating of 5 from 13 reviews, in the category Wellness center.
What does it cost?
ProVita Wellness does not publish prices on its own pages. Prices appear on its booking page, so check there before you commit. It does not publish a consultation fee either, so ask for it. The clinic does not contract with insurance, so visits are paid in cash. A superbill is available on request, which you can submit to your own insurer. It does not say whether a program price includes medication, and it does not name a weight loss, hormone or peptide product.
What should you ask before you book?
Ask which address you will be seen at. Most of the clinic's information shows the Brookfield suite, but a Wind Lake address appears in its website data, and the clinic does not explain it.
Ask which prescriber will see you and whether that person writes and signs your prescription. Ask whether a monthly fee covers the medication, and whether that medication is brand-name or compounded.
On peptides, the clinic's own words are: "Each treatment plan is customized based on an individual evaluation to ensure the most appropriate peptide protocols are used for your health goals." Ask what that evaluation costs and which peptide products it may use. Ask what follow-up visits and lab work cost, since specialty lab testing is used when appropriate but no prices are given for it.
Ask which states it can see you in by video, since it does not say. Ask how you cancel and whether there is a minimum term. Ask for office hours, which it does not list.
Who is it a good fit for?
This is a good fit if you are ready to pay cash, want a functional medicine practice with several prescribers, and will get prices, medication names and cancellation terms in writing before you pay. It is a harder fit if you need a published monthly price or a named medication before you decide.
