What does it cost?

Hormone replacement therapy and peptide therapy are each listed at $150, with "Consult Only" under the hormone price, and it does not say what that $150 covers. Botox is listed at $14.00 per unit on one price list and $12.00 per unit on another.
Membership pricing is also inconsistent. One listing shows Direct Primary Care tiers at $75, $110 and $175 a month for one person, and a second shows tiers at $75, $99 and $249. A separate Wellness Membership is $129 a month with a 3-month initial commitment. Membership is not health insurance, and it excludes labs, imaging, medications and IV infusions. The Essential Wellness tier has no long-term contract and says to cancel according to the membership agreement. The clinic is cash pay only, and it does not mention HSA or financing.
What should you ask before you book?
Ask who writes and renews the prescription, and how often you would be seen between visits. For the GLP-1 program, ask whether the $249 includes the medication, which drug it is, and whether it is brand-name or compounded. For hormone and peptide therapy, ask what the $150 covers, whether it is the consultation, the medication, or both, and which hormones or peptides would be used and how they are given.
Ask which Botox rate applies and which membership listing is current. Ask what labs, follow-up visits and medications cost on top of any membership, and how cancellation works, including whether the 3-month commitment applies to the Wellness Membership. If you live outside Wisconsin, ask which states telehealth covers, because the clinic does not list them.
Who is it a good fit for?
This is a good fit if you live in or near Baldwin, want a nurse practitioner for cash-pay primary care alongside a weight loss, hormone or peptide program, and are ready to pay separately for medication and labs. It is a harder fit if you need one fixed monthly price for weight loss or hormone care, because the clinic does not publish one.
