Transformations Inc. is a medical weight loss and cosmetic skin care clinic with six offices in West Virginia, in Huntington, South Charleston, Beckley, Parkersburg, Clarksburg and Wheeling. Joshua Galbraith, MD, founded it in 2010 and is the prescriber it names. Its weight loss program offers semaglutide and tirzepatide injections, phentermine (Adipex) and Sermorelin, which the price list calls bio-identical hormone anti-aging therapy. What does it cost?
Semaglutide is $275 for the 0.5mg weekly dose, $300 for 1mg and $350 for 2mg, each a 30 day treatment. The price list gives tirzepatide at $300 for 2.5mg, $325 for 5mg, $350 for 10mg and $375 for 15mg, but its GLP-1 section quotes $350, $375, $425 and $475 for the same doses. Its homepage advertises semaglutide from $250 a month. The office visit is $90 and includes a physician review, a monthly diet and exercise prescription, a monthly InBody scan and a monthly telehealth session with a coach. Refill and prescription support is $30. Phentermine is filled at a pharmacy you choose, and you pay that pharmacy separately. Sermorelin is $225 for a 30 day treatment, and vitamin B12 is $10.
It does not say whether the medication is included in its GLP-1 prices, whether it accepts insurance, or whether a first visit costs extra. It does not publish membership cancellation terms or prices for its skin care treatments.
What should you ask before you book?
Ask which physician will sign your prescription at the office you attend, since no prescriber is named for each location. Ask whether the monthly semaglutide or tirzepatide price includes the medication, and which price applies, since its two price lists disagree. Ask whether the injections are brand name or compounded. Ask what follow-up visits and lab work cost each month, and whether there is a first visit fee. Ask how to cancel and whether a membership has a minimum term, because neither is published. Ask for the opening hours of the office you would use.
Who is it a good fit for?
This is a good fit if you live near one of its six West Virginia offices, want physician-supervised weight loss with monthly coaching, and are ready to pay out of pocket. It also suits people in those towns who want skin treatments such as Botox or fillers alongside weight loss care. Get the prices in writing before you book, because the published numbers do not agree.
