About Medical Weight Management

What is Premier Medical Weight Management?

Premier Medical Weight Management is a physician-led medical weight loss practice in Bellevue, Washington, founded in 2018. Alexis David, MD, DABOM, DABFM, MSCP, runs the practice and prescribes. She has been board certified in Family Medicine since 2005 and in Obesity Medicine since 2018, and she is a Menopause Society Certified Practitioner. Before founding the practice, she spent 13 years as a family medicine physician.

The practice offers physician guided weight loss and medical treatments. The medications it names are phentermine (Adipex, Lomaira), Qsymia (phentermine/topiramate), Contrave (bupropion/naltrexone), Saxenda (liraglutide, aka Victoza), Wegovy (semaglutide, aka Ozempic) and Zepbound (tirzepatide, aka Mounjaro). In its own words: "Weight loss medications may be suggested or prescribed based on individual needs, but they are not required or mandatory." Visits are by appointment at 1750 112th Ave NE, Suite E-168, Bellevue, WA 98004. The phone number is 425-362-6184.

What does it cost?

The Body Composition Consult and Personalized Plan costs $97. Regular follow-up visits are $250 each. A typical six-month program has 15 visits and costs $4500 in total: $2000 for the first month, then $500 a month for months two through six. A body composition test is included in the visit fee. Payment plans are available, and vitamins and protein supplements cost extra, charged per item.

The practice does not accept insurance, and payment is due at the time of service. Dr. David is considered out of network and is not contracted with Medicare. HSA or FSA funds can be used. You receive documentation to submit to your insurer, and any reimbursement is paid to you and varies by plan. The practice does not say whether the medication is included in the program price or billed separately.

What should you ask before you book?

Ask whether the program fee includes the medication, since the practice does not say. Ask whether each prescription is the brand-name product, a generic version, or a compounded one. Ask what follow-up visits and lab work cost, and whether a refill carries a charge on top of the visit fee. Refills go through the patient portal, so ask how those are billed. Ask whether any visits can be done by video or phone, since the practice does not publish a telehealth option. Ask how a program is cancelled or paused, and whether the first month's $2000 is refundable. The practice does not publish cancellation terms.

Who is it a good fit for?

This is a good fit if you are willing to pay out of pocket, can keep in-person appointments during the office's hours, and want a physician to manage a weight loss plan that may include medication. If you need a practice that takes insurance or publishes its medication costs, ask the questions above before you call.
