About PEAK RESTORE Wellness

What is PEAK RESTORE Wellness?

PEAK RESTORE Wellness is a physician-led hormone, peptide and weight practice based at 169 N Gateway Dr., Suite 130, Providence, UT 84332. Most of its care is delivered by secure video, with labs drawn at a location near the patient. The office handles in-person SoftWave sessions for the Cache Valley area.

Two clinicians are named. Matt Hill, MMS, PA-C, is the founder and lead provider. Dr. Jeremy Broadhurst, DO, is the medical director, and he has been in full-spectrum family practice since 2009. The practice lists nine services: testosterone therapy (TRT), hormone therapy (HRT), medical weight loss, peptide therapy, sexual wellness, hair restoration, executive physicals, concierge medicine and SoftWave therapy. Medications it names include semaglutide, tirzepatide, testosterone, estradiol, progesterone, enclomiphene, Sermorelin, NAD+, tadalafil and sildenafil.

It holds a Google rating of 5 from 2 reviews. It does not list office hours.

What does it cost?

The practice is cash-pay only. In its own words: "We're a cash-pay practice by design."

A new patient consult is $199. Testosterone therapy, hormone therapy and medical weight loss are each $149 a month, and that fee does not include the medication. The Ongoing Optimization membership is also $149 a month. Peptide therapy, sexual wellness and hair restoration are $149 per visit. Concierge medicine is $300 a month, but its concierge packages are listed as coming soon. SoftWave is $125 a session, with an initial mapping special at $65. A lab follow-up visit is $85. Labs are billed to insurance or available at a cash price through the clinic. Executive physicals have no price listed.

Memberships renew under the terms shown at enrollment until cancelled. Services must be paid in full at the time of service, and refunds are not issued once services have been rendered or medications dispensed.

What should you ask before you book?

Ask who signs your prescription and who follows your plan between visits. Ask whether the $149 monthly fee includes the medication, since the listed price does not, and what the medication costs each month on top of it. Ask whether the drug is brand-name or compounded, and which pharmacy fills it, because treatments are described as pharmacy-sourced and medications ship from partner pharmacies. Ask which medications belong to each program price, and whether the $149 peptide fee is per visit or per month. Ask what labs and follow-up visits cost across a year, not just the first visit. Ask how to cancel a membership and what happens to a month you have already paid. Ask what the concierge tier includes before its packages launch.

Who is it a good fit for?

This is a good fit if you want physician-led care by video, you are ready to pay cash for consults, labs and medication, and you want a named prescriber overseeing your plan. It is a weaker fit if you need an office visit for most of your care, since SoftWave is the only service delivered on site.
