About Prime Vitality Care
What is Prime Vitality Care?
Prime Vitality Care is a physician-led practice at 10007 Huebner Rd, Bldg 3, Suite 302, in San Antonio, Texas. It is mainly a functional, longevity and aesthetic medicine practice, and medical weight loss and hormone care are two of its services. Dr. Shiv Goel, MD, is the founder and lead physician. He is an internal medicine physician certified in functional and aesthetic medicine, and he reports twenty-five years in medicine and more than 15,000 patients. The practice has a 4.8 rating from 23 Google reviews.
Its four main areas of care are medical weight loss with GLP-1 medications, hormone therapy including testosterone (TRT), peptide therapy, and a longevity membership. It also lists functional medicine, concierge medicine, IV therapy, labs, menopause management, wound care and aesthetic treatments such as neurotoxin injections and dermal fillers. Medications it names are tirzepatide, semaglutide, Ozempic, Wegovy, BPC-157 and sermorelin. Telehealth is offered to patients in Texas. Dr. Goel also writes and speaks under his own name. His author site at drshivgoel.com is separate from the clinic, and clinical visits are booked through primevitalitycare.com.
What does it cost?
The only published price is the Signature Practice Longevity Protocol, at $799 a month and capped at 20 members. It does not say whether that fee includes medication. A separate membership plan, listed in the shop section, starts at $99 a month for exclusive pricing on treatments, products and labs, and it does not explain what that plan covers. It does not publish prices for medical weight loss or hormone therapy, a consultation fee, or membership cancellation terms. It does not say whether it accepts insurance.
What should you ask before you book?
Ask who signs each prescription, and whether Dr. Goel personally writes them or other clinicians prescribe under the practice. Ask whether the $799 longevity fee covers medication, and what a weight loss or hormone program costs each month once medication is added. Ask whether the GLP-1 medicine is brand-name Ozempic or Wegovy, or a compounded version of semaglutide or tirzepatide. Ask what follow-up visits and lab work cost, whether there is a consultation fee, how many of the 20 longevity spots are open, and how a membership is cancelled. Ask whether telehealth works outside Texas, since its service area lists Texas while one of its blog posts says nationwide. Ask for office hours, which it does not list. If you want to talk before paying, it offers a membership guidance call. In its own words: "Dr. Goel reviews personally and calls within 3 business days."
Who is it a good fit for?
This is a good fit if you want a physician overseeing weight loss, hormone or peptide care, you are ready to pay cash, and you want to know who signs your prescriptions before you pay anything. It may not suit you if you need insurance billing or want published office hours to plan around.
