The pricing notes say the cost includes all visits with providers, but it does not say plainly whether the per four shot price is the whole monthly charge.

Hormone care is $99 a month, with a 6 month minimum on the monthly charge. That covers monthly consults and 15% off supplements. The initial consult is $199. Labs are $280 one time or $500 a year, with follow-up labs at $125 each for one-time lab patients. Pellets are $399 for women and $599 for men, with a 6 month minimum after each insertion. Ask which of the three providers writes the weight loss prescription. Ask whether the $165 to $255 and $215 to $425 figures are the full monthly cost, and whether they are charged per four shots, per week or per month. Ask whether the medication is always compounded and which pharmacy fills it.

Ask what follow-up visits, lab work and supplies cost on the weight loss program, since the lab prices given are for hormone care. Ask whether any visit can be done by video, because it does not say. Ask how the 6 month minimum works and what cancellation costs. The hormone terms state that a patient who leaves the program gets no refund on products or services, so ask whether weight loss follows the same rule.
