About Forte Wellness

What is Forte Wellness?

Forte Wellness is a cash-pay practice at 201 S. Calhoun St., Ste 125E, Fort Worth, TX 76104. The phone number is 817.999.1175. It offers weight loss, peptide therapy, IV infusions, vitamin injections, functional medicine, men's health and hormone optimization. Weight loss gets the most detail, naming Semaglutide, Tirzepatide, Retatrutide, Contrave and Naltrexone. In its own words: "We utilize GLP-1s in our weight loss approach."

Dr. Alex Strong, a Board-Certified Family Nurse Practitioner, owns the practice. Also listed as prescribers are Dr. Courtney Cleveland, APRN, DNP, FNP-C; Ashlyn Cuppetilli, a Registered Nurse; and Lauren Mitchell, BSN, RN.

This listing is for its Fort Worth office at 201 S Calhoun St Ste 125.

What does it cost?

Semaglutide starts with a $150 consultation that includes the first injection. After that, weekly injections are $100 each, or you can pay $500 a month up front for 5 injections and the medication. Tirzepatide starts with a $200 consultation, then $150 a weekly injection or $700 a month paid up front. Follow-ups cost $150 for semaglutide and $200 for tirzepatide, and can be done by telemedicine. Monthly packages run $575 to $650 for semaglutide and $775 to $850 for tirzepatide. A house visit adds $50.

Peptide therapy has a $150 initial consultation and $100 refill visits every 3 months. An IV infusion of 1000mL is $175. Foundations Reset, a 12-week program, is $1,900 total or $633 a month. The 6-month Longevity and Hormone Optimization program is $4,100 total or $683 a month. Men's Health is $1,500 a month. Payment is cash only. HSA cards are accepted, though the HSA provider can reject a charge later. Cancellations need 12 hour notice, and a late cancellation or missed visit costs $50.

What should you ask before you book?

Ask which prescriber will sign your prescription and who handles your follow-ups. The weight loss consultation is $50 in the FAQ but $150 on the price list, so confirm which applies. Ask whether the medication is brand name or compounded, since it does not say. Ask whether lab work is extra, because its weight loss information ties treatment to lab review but never prices it. Ask whether peptide prices include the peptide itself, and what hormone optimization covers, since no testosterone or bioidentical hormone treatment is named.

Who is it a good fit for?

This is a good fit if you plan to pay cash, want weight loss medication with follow-ups that may be done by telemedicine, and will have the prescriber, medication and lab costs confirmed in writing before you book. The clinic does not accept insurance.
