What is 61Five Health & Wellness?
61Five Health & Wellness is a multi-service clinic with offices in Nashville, Franklin and Murfreesboro, Tennessee. IV therapy leads its main page, and it also offers vitamin injections, aesthetic treatments such as Botox and fillers, and medical weight loss. Weight loss services have been offered since 2015. Patients can book appointments or walk in seven days a week. No hours are published, so ask before you make the trip.
Weight loss is the part most relevant here. The program offers GLP-1 medications semaglutide, tirzepatide and retatrutide, all compounded, plus phentermine and lipotropic (MIC) injections. Dr Moses Swauncy, MD is the one physician named as overseeing the program and as its prescriber. Google shows a 4.9 rating from 1231 reviews. In its own words: "At 61Five Health & Wellness, we don't focus exclusively on medication for weight loss assistance."
This listing is for its Franklin office at 625 Bakers Bridge Ave #110.
What does it cost?
The GLP-1 injections are priced per injection and calculated by dose. Semaglutide, sold as the GLP-1 SMA injection, runs $75 to $129 per injection. Tirzepatide runs $80 to $160, and retatrutide runs $99 to $179. It does not say whether the per-injection price includes the medication. Packages that include medication and weight loss injections start at $99, according to its FAQ. Phentermine is $50 for a new patient and $75 for returning patients, and that price includes a B12 injection. Lipotropic packages are $49 for four injections and $89 for eight. Lipo-MAX packages are $59 for four and $95 for eight. Initial weight loss consultations are free. The clinic accepts HSA and FSA. It does not say whether other insurance is accepted, and it does not mention financing. In its own words: "There are no commitments or fees when starting our program."
What should you ask before you book?
Ask these before you pay anything. The main line is 615.401.9380. Start with who signs your prescription. Dr Moses Swauncy, MD is named as the prescriber, but the weight loss team is described as nurse practitioners, and it does not say which providers can prescribe the injections. Ask whether the per-injection price covers the medication. Ask what compounded means for the product you would receive, and whether the clinic can pause or change a medication partway through a program. Ask what follow-up visits and lab work cost, and what your total each month comes to once phentermine, injections and visits are added together. Ask whether visits can be done by video, since that is not stated. Ask how to stop, and whether any fee applies after your first visit. Ask for the hours at the office you would use. Finally, ask whether any insurance beyond HSA and FSA is accepted.
Who is it a good fit for?
This is a good fit if you want medication-based weight loss at one of the three offices, you will pay out of pocket or with HSA or FSA funds, and you want the prescriber and the monthly total confirmed in writing before you commit. If you need video visits, a named prescriber on your paperwork, or published hours, get those answers first.
