What is Lake Med?
Lake Med is a primary care and wellness clinic at 1852 Decherd Blvd in Decherd, Tennessee. Most of its work is everyday primary care: routine and sick visits, chronic disease management, and physicals, including DOT physicals for commercial drivers and school and sports physicals. It serves individuals, families, students, employers, and commercial drivers in Decherd and nearby communities. Weight loss is one service among several, alongside IV hydration therapy, diagnostic testing, and women's and men's health care that includes hormone evaluations, hormone replacement therapy, and testosterone evaluations. Peptide therapy is not among its listed services.
The weight-loss program covers medical evaluation, nutrition, activity and behavior planning, and monitoring of progress, side effects, and lab work. The medications it names are Wegovy, Zepbound, Foundayo, Saxenda, Qsymia, Contrave, orlistat products, and phentermine. In its own words on compounded drugs: "Compounded medications are not FDA-approved and are not automatically interchangeable with an FDA-approved product." The clinic does not name the clinician who prescribes weight-loss medication or hormone therapy. It offers telehealth for appropriate follow-up and concerns. It is open Monday through Thursday from 8:00 AM to 4:30 PM and Friday from 8:00 AM to 12:00 PM, with lunch closed from 12:00 PM to 1:00 PM daily. The phone number is 931-998-2915. It is rated 4.8 on Google from 19 reviews.
What does it cost?
The one published price is the membership: $75 a month for ages 18 to 25 and $125 a month for adults 26 and up. There are no contracts, and you can cancel anytime. The membership covers unlimited office visits, telehealth visits, an annual wellness exam, chronic disease management, point-of-care testing, and basic labs. It does not cover hospital or emergency care, specialists, imaging, upgraded labs, or medications. The membership is not health insurance.
It does not publish prices for weight-loss programs or hormone therapy. It does not say whether medication is included in any price, whether insurance is accepted, or what a consultation costs. Ask about all three before you book.
What should you ask before you book?
Ask who signs the prescription and what credentials that person holds. Ask whether a program fee includes the medication or whether you pay for it separately at pharmacy prices. If a medication is compounded, ask whether it is a compounded version rather than the brand-name product, and where the pharmacy sources it. Ask what the consultation, follow-up visits, and lab work cost each time, and whether they are billed to insurance or paid in cash. Ask how you cancel a weight-loss program, since cancellation terms are given only for the membership, and whether there is a fee.
Who is it a good fit for?
This is a good fit if you want primary care close to home in Decherd, and you want weight-loss or hormone questions handled in the same office. It is a weaker fit if you need the full cost and the prescriber named before you call. Call 931-998-2915 and ask those questions first.
