About Elemental Health & Wellness
What is Elemental Health & Wellness?
Elemental Health & Wellness is a direct primary care practice at 221 Scranton Carbondale Hwy, Scranton, PA 18508. The town is given two ways: the street address is in Scranton, but Dickson City also appears as the town. Appointments are required, and the phone number is 570-689-1866.
Jessica Washenko-Blank, MSN, NP-C, is the founder and lead practitioner and the only prescriber named. The core of the practice is subscription primary care, with physicals and a flat-rate urgent care visit. The wellness side covers medical weight loss using semaglutide and tirzepatide, hormone care (TRT, BHRT, thyroid and adrenal support, and menopause care), peptide protocols, anti-aging care, and IV infusions. The clinic offers eleven peptides, including BPC-157, sermorelin and NAD+. Virtual visits are offered to patients in PA and NY. It does not state how long it has been open.
What does it cost?
The $250 initial consultation is a one-time fee that comes before any ongoing program. Weight loss runs $99 a month for home self-injection, plus the cost of medication billed separately. In-office semaglutide is $329 a month with medication included, and in-office tirzepatide is $429 a month with medication included. Men on metabolic optimization pay a $129 monthly membership, and all others pay $125 a visit. Menopause and peptide follow-ups are $125 a visit. Peptide pricing varies by protocol and is set at consultation, so no peptide price is published. Primary care prices are not published either.
Labs cost $450 cash if drawn in the office, or can be run through insurance at an outside lab. In its own words: "Lab work is not included in any membership pricing." High-dose vitamin C starts at $159, and vitamin injections start at $25.
All services are self-pay, and insurance is not accepted for its programs. Cancellation terms, financing, and HSA use are not described.
What should you ask before you book?
Ask whether Jessica Washenko-Blank personally reviews and signs each prescription, and who handles your care when she is unavailable. Ask whether the $329 and $429 figures are your full monthly bill, and what the total comes to on the $99 home plan once medication is added.
Ask whether the medication is brand-name or compounded, and which pharmacy supplies it. For peptides, ask which of the eleven have approval for the use you have in mind, and what a full protocol costs each month before you commit.
Ask whether repeat labs and follow-up visits are included in the monthly fee, and what they cost if not. Ask how the primary care waitlist works and how long it runs. Ask how cancellation works and whether there is a minimum commitment.
Finally, ask which Friday hours are correct, since both 10 to 3 and 9 to 3 are given, and confirm which town the office is in.
Who is it a good fit for?
This is a good fit if you want cash-pay primary care or a monthly weight-loss, hormone or peptide program, and you can pay lab costs and recurring fees up front. It is a weaker fit if you need insurance to cover the visit, or if you need a primary care appointment soon, since that panel is currently full.
