About Hometown Primary Care

What is Hometown Primary Care?

Hometown Primary Care is a direct primary care practice at 13511 NE 23rd Street, Suite 140, in Choctaw, Oklahoma. It opened in November 2022. Candace Herrin, APRN, FNP-C, is the only prescriber named, and she founded the clinic. Patients pay one flat monthly membership fee for unlimited primary care visits, same-day or next-day appointments when available, and direct phone, text, and email access. The clinic serves Choctaw, Eastern Oklahoma County, and the wider OKC metro, with telemedicine across Oklahoma and Florida.

Primary care is the main business. Medical weight loss and hormone replacement therapy are added services, along with Triton laser hair removal, EmpowerRF treatments, and IV infusions and injections. The weight loss program uses the GLP-1 medications Semaglutide and Tirzepatide, which belong to the same classes of medication as Ozempic, Wegovy, Mounjaro, and Zepbound. Hormone therapy is offered to women and men and uses bioidentical hormones by cream, patch, injection, or pill. The clinic does not offer pellet therapy. It holds a 4.9 rating on Google from 67 reviews. Hours are Monday through Friday, with Fridays by appointment only.

What does it cost?

Membership is the one set price. Adults ages 18 to 54 pay $80 a month, adults 55 and up pay $90, and each child under 18 pays $40. A family of four (two adults, two children) pays $220 a month. A single-parent family (one adult, two children) pays $150, and each additional child adds $35. There is a one-time $50 enrollment fee. Membership runs month to month with no long-term contract. Annual prepay is available with savings, but the clinic does not state the amount. A non-member visit costs $120. The first meet and greet is free and runs 15 minutes. The clinic is cash pay only. In its own words: "There is no insurance billing, no copays, and no surprise bills."

Medical weight loss and hormone therapy have no published prices. Weight loss pricing depends on the program tier you choose. Hormone therapy is priced separately from membership at the standard non-member rate. The clinic does not say whether medication is included in any price, or whether labs are part of membership. Members get wholesale pricing on labs and common medications.

What should you ask before you book?

Ask whether any medication is covered by the monthly fee, and what a month of Semaglutide or Tirzepatide costs on top of it. Ask whether the medication is brand name or compounded, and who supplies it. Semaglutide is described as a more accessible option than brand-name pricing, but the clinic does not say how the drug is made or sourced. Ask what each weight loss tier includes and costs, and whether follow-up visits and dose adjustments are part of it. Ask what lab work costs, since the clinic does not say labs are included in membership. For hormone therapy, ask what the in-person first visit and labs cost, because an initial in-person visit is typically required before treatment begins. Ask how cancellation works after the first month, and whether the $50 enrollment fee is refundable. Finally, ask whether Candace Herrin will be the provider you see.

Who is it a good fit for?

This is a good fit if you want a primary care practice you can reach directly and you are ready to pay cash for it, or if you live in Choctaw or Eastern Oklahoma County and want weight loss or hormone care from the same practice. If you need the full monthly cost, including medication, before you commit, get those answers by phone before you book.
