About AfyaGM Health & Wellness

What is AfyaGM Health & Wellness?

AfyaGM Health & Wellness is a Direct Primary Care practice at 8919 Brookside Ave, Suite 105, in West Chester, Ohio. Patients pay a flat monthly membership, and the practice does not bill insurance for primary care visits. Dr. Tamara "Tomi" Small, APRN, FNP-C, MSN, PhD, is the prescriber. She founded the practice and has more than 25 years of experience.

Primary care is the core of the practice. It also offers medical weight management and hormone replacement therapy, each run as a program on top of membership, along with DOT physicals and TB testing and screening, mostly for employers. It does not list peptide therapy or longevity medicine.

Members can visit the West Chester office in person or book virtual visits for patients in Ohio, Arizona, Minnesota and Washington, which are included in the membership. Dr. Small is licensed in all four states. The practice has a 5-star rating on Google from 5 reviews.

What does it cost?

An individual membership is $89 a month, a couple is $159 a month, and a family of two adults and two children ages 12 to 17 is $229 a month. Each additional child adds $30 a month and each additional adult adds $49 a month. Every membership carries a one-time $50 enrollment fee. Paying annually takes 10% off. Memberships are month to month, and you cancel with 30 days notice.

The hormone and weight programs are harder to pin down. The practice gives Balance+ DPC a price of $239 a month for hormone therapy, yet elsewhere it says Balance+ is included with membership at no additional cost. Vitality+ DPC, the weight management program, has no published price. The practice does not say whether medication is part of any of these prices, and it publishes no consultation fee. Lab work is called low cost, but no figures are given. Specialist referrals, imaging and hospital care are billed separately, through insurance or self-pay. The practice says HSA funds can be used for memberships starting in 2026, and suggests confirming with your HSA administrator.

What should you ask before you book?

Start with Balance+. Is the $239 a month charged on top of membership, or is the program included? Get the answer in writing, since the practice describes it both ways. Then ask whether the monthly fee covers hormone or weight-loss medication, and which medications. The practice mentions a medication program through Manifest Rx with over 1,000 medications at no cost, but it does not say whether that covers hormone or weight management drugs. It names no weight-loss medications, so ask which ones it uses and whether they are brand-name or compounded.

Next, ask for dollar figures on the Vitality+ program, lab work and follow-up visits. Ask whether insurance can be billed for hormone or weight care, since the practice only says it does not bill insurance for primary care. Ask who prescribes when Dr. Small is away, what the office hours are, and whether the $50 enrollment fee is refunded if you cancel early.

Who is it a good fit for?

This is a good fit if you want primary care with one consistent provider, in person or by video in four states, and you are ready to pin down hormone or weight management costs before you enroll. If you are looking for peptide therapy or longevity medicine, the practice does not list either.
