What is Foraged Health LLC?

Foraged Health LLC is a functional medicine practice with three sites in Tuscarawas and Holmes Counties, in Uhrichsville, Zoarville and Sugarcreek, Ohio. All three are by appointment only. Much of what it offers is lab testing and IV or injection therapy, aimed at hormones, gut health and energy.

Alexis Manin, APRN, FNP-C, MSN, is the founder and lead provider and a nurse practitioner. Registered nurses Cassandra Brown and Olivia Wagner also work with clients, and Olivia Wagner provides IV hydration and wellness services. Foraged Health names HRT, TRT, B12, glutathione, NAD+ and GLPs among its treatments. Peptides appear only as an IV add-on. Its Google listing shows a 5 rating from 11 reviews, in the category Wellness center.

What does it cost?

Memberships are $99 a month (Holistic Health), $149 a month (Functional Health) or $399 a month (Concierge Health). Each has a 6-month minimum commitment and a card on file, billed monthly. Recurring prescriptions require Functional Health or Concierge Health, and pay-as-you-go pricing is not available for them. Medication is billed separately from the membership fee, and so is lab testing unless stated otherwise.

Individual services are priced from these starting points: IV hydration at $125, personalized IV therapy at $160, consultations at $225, functional lab testing at $499, and GI-MAP or DUTCH hormone testing at $599 each. Vitamin and wellness injections start at $25. Foraged Health does not publish a price for hormone replacement therapy or weight loss medication. It does not say whether insurance is accepted for visits, only that Functional Health members using insurance may be given conventional lab orders so they can use their insurance benefits.

What should you ask before you book?

Ask who writes and signs your prescription, and how often you will need follow-up visits. Ask whether a membership guarantees any prescription, since the membership terms say it does not. Ask what the medication costs each month, and whether it is brand-name or compounded, because neither is stated. Ask what labs your plan needs, what each one costs, and whether any of them can go through your insurance. Ask what the weight loss program includes and what a peptide add-on contains. Ask how to cancel after the 6-month minimum, and whether telehealth or posted hours are available, since neither is listed.

Who is it a good fit for?

This is a good fit if you want lab testing, IV or injection therapy, or hormone care near Uhrichsville, Zoarville or Sugarcreek, and you are ready for a monthly bill with a 6-month minimum. It may not fit if you need a fixed price for medication, or written answers on cancellation and telehealth before you pay.
