About Simplicity Health Direct Primary Care

What is Simplicity Health Direct Primary Care?

Simplicity Health Direct Primary Care is a direct primary care practice at 200 East Spring Valley Pike, Suite A, in Centerville, Ohio. Its main work is primary care, paid for by monthly membership. Four physicians prescribe there: Drs. Opperman, Marton, Butler and Wenzell. You are matched with one of them and see that doctor at each visit. Weight loss and hormone therapy are two of its services. Weight loss is offered as GLP-1 therapy, and hormone therapy includes testosterone replacement therapy and hormone replacement therapy for women. The practice also lists diabetes, hypertension, mental health, women's health and pediatric care. It has an in-house lab and pharmacy, and visits run 30 to 60 minutes. It names Springboro, Kettering, Oakwood, Bellbrook, Miamisburg, Beavercreek, Lebanon and Waynesville among the communities it serves. Its Google rating is 4.9 from 82 reviews.

In its own words: "When GLP-1 medication is the right call, we prescribe and manage it carefully using FDA-approved, branded medications dispensed through proper pharmacy channels."

What does it cost?

Gold is $79 a month, or $49 a month for adults under 40. It includes one office visit a year, and additional visits are $50 each. Platinum is $129 a month, with unlimited 30-minute visits and after-hours phone access for urgent or emergent issues. Diamond is $199 a month, with unlimited 60-minute visits, most labs and medications included, and up to four house or work calls a year. Children under 18 are $49 a month with an enrolled adult. Paying annually saves about 10%. The first visit is free.

Members keep their insurance, but the practice does not bill it, so the membership is paid out of pocket. Gold and Platinum fall within federal HSA limits, and Diamond's HSA payment is worked out at the meet and greet. FSA eligibility depends on the plan. There are no contracts and no enrollment fees, and you can cancel anytime. If you cancel mid-year on annual billing, the refund is prorated. The practice does not publish a price for GLP-1 or testosterone medication, and its membership pricing does not say whether those drugs are included at Gold or Platinum.

What should you ask before you book?

Ask which of the four physicians would prescribe for you and whether you will see the same one each visit. Ask whether GLP-1 or testosterone medication is included in your monthly fee or billed separately, and what it costs each month. Ask for the brand name of the medication, and whether any part of it is compounded. The practice describes its medications as branded, so get that confirmed in writing.

Ask what follow-up visits and lab work cost on the tier you are considering. Ask which states telehealth visits cover, because the practice does not name them. Ask whether there is a waitlist, since it says panels fill quickly and then move to one. Finally, ask how cancellation works and how a prorated refund is calculated if you pay annually.

Who is it a good fit for?

This is a good fit if you want one physician who sees you at every visit, you can pay monthly out of pocket for primary care, and you are comfortable asking for medication prices before you commit. It is a weaker fit if you need the medication price in writing before you call, or if you live outside the Dayton area and want telehealth in a state the practice has not named.
