What is Kultivate Women's Health?
Kultivate Women's Health is a women's health practice in Beachwood, Ohio, run by nurse practitioners and focused on women in perimenopause, menopause and midlife. Its prescribers are Ann Konkoly, APRN, who founded the practice, Holly DeWalt, APRN, Jean Marino, APRN, and Shaylinn Romano, APRN.
The practice lists six service lines. Two of them fit this directory: hormone care using estradiol, progesterone and testosterone, and medical weight loss using GLP-1 medications. The weight loss program uses compounded semaglutide or compounded tirzepatide filled through Akina Pharmacy, or brand-name Zepbound ordered through LillyDirect. In its own words: "We take a thoughtful, science-driven approach using GLP-1 medications when appropriate, metabolic testing, nutrition support, and personalized care."
Visits take place at the Beachwood office or by telehealth for patients in Ohio, New York and Arizona. The only hours published are Friday, 09:00 to 14:00. Its Google listing shows a rating of 5 from 49 reviews.
What does it cost?
The compounded weight loss programs are monthly memberships. The semaglutide program is $275 a month and the tirzepatide program is $425 a month. Both include the medication, syringes, two 30-minute follow-up visits a month and InBody scans. The minimum commitment is three months. After that, you may cancel with 30 days' notice.
The KWH Support Membership is $225 a month, also with a three-month minimum. It covers provider visits, InBody scans, coaching and medication guidance, but its description does not name medication. If you choose Zepbound, you pay LillyDirect directly: $349 a month for the 2.5 mg dose, $499 for 5 mg, and $499 to $699 for 7.5 to 10 mg, depending on refill timing.
The practice does not accept insurance, so all care is paid out of pocket. It does not publish hormone therapy prices or a consultation fee.
What should you ask before you book?
Ask which prescriber you would see, and who manages your hormones after the first visit. Ask whether the $225 support membership includes medication, since its description does not say. Ask whether the hormones prescribed are FDA-approved or compounded, and what lab work and follow-up visits cost. Because hormone prices and consultation fees are not published, ask for both before you book.
Refills are not automatic. Ask who tracks your supply and how far ahead you need to message the office. Ask how cancellation works if you want to leave before the three months are up, and what happens to medication already shipped to you. If you live outside Ohio, New York or Arizona, ask whether telehealth is available to you at all. You can call the office at (216) 245-3254.
Who is it a good fit for?
This is a good fit if you live in Ohio, New York or Arizona, want cash-pay care for midlife hormones or medical weight loss, and can commit to a monthly membership with a three-month minimum. If you need care billed to insurance, this practice does not accept it.
