What is Concinnity Health?
Concinnity Health is a hybrid direct primary care practice at 258 Southtown Circle, Rolesville, NC 27571, reached at (919)435-1185. Primary care is its main work, and weight loss is one of its services. That service covers personalized plans, dietary counseling, follow-up monitoring and prescription weight loss medications. The practice also offers chronic disease management, men's and women's health services, and DOT physicals. No prescriber is named, and no credentials are given for the team. It does not say whether hormone therapy or peptide therapy is offered. Hours are Monday through Thursday, 9:00am to 6:00pm, and Friday, 9:00am to 12:00pm. It serves Rolesville, Wake Forest, Raleigh, Zebulon, Knightdale and nearby areas. It has a 4.8 rating on Google from 24 reviews.
What does it cost?
Patients without insurance pay a flat monthly membership: $55 a month for ages 5 to 19 ($45 with an adult membership), $95 a month for ages 20 and up, and $245 a month for a family plan covering two adults and up to two dependents under 20. Additional dependents are $35 each. A one-time $99 enrollment fee starts the membership, which includes unlimited primary care office visits, or virtual visits, with no copays.
Insured patients are not offered membership. In its own words: "we are not able to offer membership to anyone who is insured by those we are in network with." The practice says it is in network with most major insurers and lists NC Medicaid and Medicare. The monthly fee does not say whether weight loss medication is included, and no weight loss program price or consultation fee is published. Membership has no contract, but cancelling takes 30 days' written notice, and a $150 re-enrollment fee applies if you come back.
What should you ask before you book?
Start with who writes the weight loss prescription and what their credentials are, since no prescriber is named. Then ask whether weight loss medication is in the monthly fee or billed on top, and whether it is brand-name or compounded. Ask what the program costs in total, and what follow-up visits and lab work cost beyond membership.
If you have insurance, ask whether your plan covers weight loss medication, since insured patients cannot join the membership. Ask whether hormone therapy or peptide therapy is offered and who would provide it. Before paying the $99 enrollment fee, ask for the membership agreement, because the cancellation terms refer to it.
Who is it a good fit for?
This is a good fit if you have no insurance and want a flat-fee primary care office with no-copay visits, and you also want weight loss care in the same practice. If you need a named prescriber or a published weight loss price before you commit, call first and get those answers in writing.
