About AG Medical Wellness

What is AG Medical Wellness?

AG Medical Wellness is a direct primary care practice at 200 S. Broadway, Suite 2-6, in Tarrytown, New York. Its main business is primary care for the whole family, from annual physicals and sick visits to chronic disease management and women's and men's health. Weight management and physician supervised weight loss is one service on that list, not the center of the practice.

Dr. Ana Guerra, MD, a board-certified Family Medicine physician with over 16 years of clinical experience in New York City and Westchester County, is the only prescriber named. She trained at NYU Lutheran Medical and is fluent in Spanish. The practice offers virtual care when appropriate, same or next day, and home visits for an additional cost. It has a Google rating of 5 from 16 reviews.

The practice does not name any weight loss medication, and it does not mention hormone therapy or peptide therapy. It does not publish office hours either, so call (914) 506-9573 to ask.

What does it cost?

Membership is the core of the model. Individual membership is $129 a month, plus a $100 enrollment fee that is non-refundable. That rate is introductory until September 1st, 2026, after which it is $149 a month. Children ages 5 to 17 are $50 a month with a parent enrolled, and couples and families pay a $150 enrollment fee. Members pay no copays and no per visit charges, though the practice mentions a nominal fee for some in-office procedures.

Non-members pay $275 per office visit. Home visits are $200 per visit for members living within 5 miles of the practice. The practice accepts select commercial insurance for traditional office visits and accepts Medicare for fee-for-service office visits. It does not publish a consultation fee or a price for its weight loss program, and it does not say whether medication is included. In its own words: "We ask for 30 days' notice for membership cancellation, which gives us time to prepare you for a smooth transition to a new practice."

What should you ask before you book?

Ask who writes a weight loss prescription, and whether Dr. Guerra does it herself. Ask whether the monthly fee includes any medication. The practice says members can use their insurance for labs, medications, imaging and specialty care, so ask whether a weight loss drug would be billed to insurance or paid by you. Ask whether the medication is brand-name or compounded, and which pharmacy fills it.

Ask for the full cost of the program in writing, including follow-up visits and lab work. The practice mentions discounted cash prices for labs but gives no figure, so ask for that number. Ask what the in-office procedure fee is before you agree to anything. Finally, confirm the cancellation terms, since the practice asks for 30 days' notice.

Who is it a good fit for?

This is a good fit if you want a family doctor who handles your primary care and can also discuss physician supervised weight loss, and you are comfortable paying a monthly membership before the weight loss pricing is confirmed. It is a weaker fit if you want a weight loss program with named medications and published prices before you book, since the practice publishes neither.
