Legend Elevate is a medical weight loss clinic with offices listed in Pearl River, NY and Clifton, NJ. Its online materials also name Suffern, NY as a service area. Medical weight loss is the main service it advertises, and it also offers peptide therapy, TRT, Botox and IV therapy.

What does it cost?
The clearest figure is a 10-week medically supervised program at $500, which includes the prescribed weight loss medication. Its breakdown shows $234 for medication paid to the pharmacy, $13 for pharmacy shipping and $253 for medical oversight, follow-ups and monitoring. In its own words: "The remaining program balance covers professional medical services provided throughout the 10-week treatment period."
Other plans are $278, $210, $1,080 for six months, $1,530 for nine months, and $460 on a plan titled $450. Two three-month plans are $205 a month and $216 a month. The six-month plan says medication is paid to a third-party pharmacy, so it is not in the price. For most other plans the clinic does not say whether medication is included. You can pay in full or choose a payment plan. The clinic does not publish a consultation fee, an insurance policy, HSA or financing options, or cancellation terms.
What should you ask before you book?
Ask who writes the prescription. No prescriber is named, and the clinic's name does not tell you who signs. Ask whether the $500 program's price covers the medication, and whether the other plans do too. Ask whether the medication is brand-name or compounded, and what it costs each month. Ask what the disclaimer means in practice. In its own words: "Medication is prescribed only if medically appropriate after evaluation." Ask what that evaluation includes and whether it costs extra.
Ask what lab work costs, since some plans include blood work and others mention lab review only when needed. Ask whether follow-up visits cost more after a program ends, and whether insurance is billed at all. Ask how cancellations and refunds work once a payment plan starts.
