About Introducing MDVIP
What is this practice?
This is a medical office at 4515 Military Road in Niagara Falls, New York, run by Dr. Todd M. Orszulak, D.O., and Maureen Westgarth, F.N.P. The website's page titles read "Introducing MDVIP," but no other name appears for the practice, so search by the address or the provider names. It is open Monday through Thursday, 7am to 5pm, and the phone number is 716.236.8960. Dr. Orszulak has been in private practice at this location since 2009, and the provider biographies describe primary care work.
The practice lists four services. Medical weight loss uses semaglutide (sold as Ozempic and Wegovy) or tirzepatide (Mounjaro and Zepbound), given by weekly office injection for the first 16 weeks along with medical monitoring, body composition analysis, and diet and exercise coaching. The BioTE hormone program uses custom-compounded pellets containing testosterone and estrogen, placed under the skin, and its BioTE page also has a section on prescription peptide therapeutics. The practice also offers Botox treatments and fillers, plus an online supplement dispensary through Fullscript. Google lists the practice as a medical clinic with a rating of 2.3 from 3 reviews, which is a small sample.
Maureen Westgarth's biography describes a separate family business in Canada, ChiroThin, which it says is offered to American patients by televisit. The footer also links to a "ChiroThin Weight Loss Program." The practice does not explain how that program relates to the in-office injections.
What does it cost?
The initial consultation is $99.00. The practice says that once the consultation is complete and you are approved for treatment, your first injection may be given the same day. It also mentions free consultations for MDVIP members and a "2 for 1" offer on initial consultations, but it does not say what membership costs or who qualifies.
Weight loss medication is priced per month and rises at each stage. Semaglutide runs from $275 a month in the first four weeks to $375 a month from week 17 on. Tirzepatide runs from $350 to $750 a month over the same schedule. The table is headed "Total cost per month," but the practice does not say what that covers. On payment, the practice says: "Each month must be paid in full before the first dose is given." It does not publish prices for hormone pellets, peptides, or Botox, and it does not say whether it accepts insurance or how cancellation works. Its hormone and Botox sections say to call for a consultation.
What should you ask before you book?
Ask who signs each prescription and which provider you will see for follow-up. Ask whether the monthly weight loss fee includes the medication, and whether the medication is brand-name or compounded, since the practice says compounded semaglutide or tirzepatide may be used. Ask what lab work, follow-up visits, and visits after week 16 cost.
Ask how cancellation works and whether a month already paid is refunded, given that payment is due before the first dose. For the ChiroThin televisit, ask whether it is the same program as the office injections, who prescribes it, which states each prescriber is licensed in, and whether it is available to you in New York. The practice does not name the states it serves for telehealth. For hormone care, ask what the initial lab testing, pellets, and follow-up cost. For peptides, ask which ones are offered, who prescribes them, and what they cost.
Who is it a good fit for?
This is a good fit if you live within reach of Niagara Falls, can attend weekly office visits during the first 16 weeks, and are comfortable paying each month in full before the first dose. If you are mainly interested in a televisit, ask the ChiroThin questions before you book.
