About Evolve Integrative Health

What is Evolve Integrative Health?

Evolve Integrative Health is an integrative and functional medicine practice at 2 Normanskill Blvd, Suite 206, in Delmar, New York. The phone number is (518) 394-1043. Lisa Preller, MS, CNM, RN, LM, founded the practice, which launched in February 2025. She is the only person the clinic names as a prescriber. She also founded Empowered Midwifery Care PLLC in 2021. Giselle Ginsberg, MS, RD, is a registered dietitian on the team who supports weight care, but she is not listed as a prescriber.

The clinic offers a general integrative health visit, Hormone Optimization, IV Nutrient Therapy, Weight & Metabolic Health, Aesthetic Therapies, and Neuromodulator Treatments. Its weight program uses GLP-1 receptor agonists. Botox® and Jeuveau® are given in its aesthetic service by a CRNA. In its own words: "Led by Lisa Preller and a collaborative care team, we provide unhurried, individualized care focused on root-cause healing and sustainable wellness." Its Google listing shows a rating of 5 from 3 reviews and a category of medical spa.

What does it cost?

The 60-minute initial visit starts at $250 for the general integrative health service. It starts at $275 for Hormone Optimization and for Weight & Metabolic Health. The 30-minute follow-up visits start at $125 for the general service and at $150 for hormone and weight care. IV infusions are priced by formula, from $125 to $250 each. Neuromodulator pricing varies. The clinic does not publish prices for GLP-1 medication or hormone therapy. It does not say whether medication is included in the visit price, whether it accepts insurance, or how cancellation works. A grant from the Albany County Legislature funds a community-supported scholarship fund for patients who need financial help with services.

What should you ask before you book?

Ask who writes and manages the GLP-1 prescription. Lisa Preller is the only prescriber the clinic names, and it does not say who handles weight medication. Ask whether the medication is included in the $275 visit price or billed separately, and what it costs. Ask whether it is brand-name or compounded.

Ask what follow-up visits, lab work, and hormone testing cost in total. Ask whether it takes insurance, offers telehealth, and what its hours are. Ask how to cancel. Ask which of Lisa's listed certifications are still pending, since her bio lists a NAMS certification as pending June 2025 and an A4M fellowship as pending December 2025. Ask for the current written agreement before you pay, because the online terms of use name a similar but different business and web address.

Who is it a good fit for?

This is a good fit if you want an integrative visit with a midwife-trained founder for hormone or weight concerns, you can pay cash, and you want medication prices in writing before you book. It may not suit you if you need insurance billing, published medication prices, or a named prescriber for weight care. Call first and ask those questions.
