What is MD365?
MD365 is a primary care and multispecialty clinic founded in 2019, with offices across Long Island and one in Yonkers. It has offices in Brentwood, Centereach, East Moriches, Lake Ronkonkoma, New Hyde Park, Oakdale, Patchogue, Shirley, Syosset and Yonkers. The newest office is in Brentwood, and the main phone line is (631) 400-8400.
Primary care is the core of the practice. Walk-in care, pediatric care, women's health, cardiology, vascular medicine, neurology, immigration physicals, DOT physicals, behavioral health, vaccines and diagnostics are also offered. Weight loss is one service among these. The clinic offers GLP-1 medications, names semaglutide and tirzepatide, and lists the brands Ozempic, Wegovy, Mounjaro and Zepbound. It also offers a virtual weight loss consultation. It does not list hormone therapy or peptide therapy.
The providers named include physicians, nurse practitioners and physician assistants, among them Jacob Mathew, MD, Marc Cimmino, DO and AnnaMaria Pepitone, FNP. In its own words, the weight loss care runs "under the supervision of our board-certified doctors."
What does it cost?
MD365 does not publish a price for GLP-1 weight loss, and it does not say whether medication is included in any price. No consultation fee is listed. The clinic claims "the most competitive price in New York State," but gives no figure to check that claim against.
MD365 accepts most insurances and names Fidelis, WellCare and Healthfirst, along with most major commercial, Medicare and Medicaid managed care plans. It also runs a Medicare GLP-1 Bridge Program for eligible Medicare patients, with help on eligibility and prior authorization. Membership terms, cancellation rules, and HSA or financing options are not stated.
What should you ask before you book?
Ask who signs the GLP-1 prescription and what credential that person holds, since the providers include both physicians and non-physician clinicians. Ask whether the program fee includes the medication or whether the drug is billed separately. Ask whether the medication is the brand-name product or a compounded version. The clinic names brands and active ingredients without saying which it dispenses.
Ask what the virtual consultation, follow-up visits and lab work cost, and whether a visit is charged per appointment or per month. If you have Fidelis, WellCare, Healthfirst or Medicare coverage, ask whether it covers the medication and who handles prior authorization. Ask how you cancel and whether there is a membership term. Ask which office you would be seen at and what its hours are, because none are published. If you want hormone or peptide therapy, ask directly, since the clinic does not mention either.
Who is it a good fit for?
This is a good fit if you want primary care and GLP-1 prescribing under one roof at a Long Island or Yonkers office, and you are willing to get the price and the prescriber's name in writing before you book. If you need a published price to compare before you call, this clinic will not give you one.
