About FourC Health
What is FourC Health?
FourC Health is a Direct Primary Care clinic at 8224 Louisiana Blvd NE, Suite C, Albuquerque, NM 87113. Patients pay one flat monthly membership instead of billing insurance per visit, and medical weight loss runs under that same membership. The clinic names one prescriber, Leena Cherukuri, MD, MPH. She is board-certified in Family Medicine and Obesity Medicine, completed a Family Medicine residency at UNM in 2009, and led the non-surgical medical weight loss clinic at RUST Presbyterian Medical Services from 2022 to 2025. The clinic describes 18+ years of experience in primary care, weight management and hospital medicine. Its Google listing shows 5 stars from 59 reviews.
Other services include preventive exams, chronic condition management, sick visits, InBody body composition scans, in-clinic tests and minor procedures, video or phone visits after the first in-person visit, and direct messaging with the doctor through its app. Among the GLP-1 medicines it names are Ozempic, Wegovy, Mounjaro and Zepbound. In its own words: "No compounded meds, no peptides." It is open Monday to Friday, 8 am to 12 pm and 1 pm to 5 pm, and closed on weekends.
What does it cost?
The membership is $99 a month, plus taxes, and requires a one-year commitment. The clinic offers a free doctor intro call before you sign up. The membership covers visits, messages, InBody scans, and the in-clinic tests and procedures it lists. It does not say whether medication is included in the $99. Its primary care page lists prescription costs as outside the membership, and it does not publish medication prices, including for the GLP-1 medicines it names. It does not bill insurance for membership or in-clinic visits, and it does not serve Medicare, Medicaid or TRICARE patients. External lab tests, imaging, specialist visits and hospital care are not covered by the membership. The clinic also advertises a $75 trial with cancellation inside the first month, which sits alongside the one-year term.
What should you ask before you book?
Start with whether the $99 includes the medication, or whether prescriptions are billed on top at your pharmacy. Ask which GLP-1 medicine the doctor would prescribe, whether it is the brand-name product or a generic version, and what it costs each month. Ask how the $75 trial fits with the one-year commitment and what it costs to leave early. Ask what follow-up visits and lab work cost beyond the membership, and what side effects to watch for and when to call the office.
Who is it a good fit for?
This is a good fit if you want one physician for ongoing primary care and weight management, can pay $99 a month plus taxes on a one-year term, and are ready to cover medication costs yourself. It is less suited to people who need Medicare, Medicaid or TRICARE coverage, or who want a single visit without a membership.
