About Achieve Health NV
What is Achieve Health NV?
Achieve Health NV is a direct primary care and weight management practice at 6490 McCarran Blvd. Bldg. A #3 in Reno, Nevada. Its physicians are Sara Trigero, MD, and Amy Sanguinetti, MD, PhD. Both are dual board-certified in Internal Medicine and Obesity Medicine. The practice also lists Lifestyle Medicine and Menopause Certified among its credentials.
It offers weight management for adults and adolescents age 16 and up, primary care for adults, women's health and hormone replacement, men's health, acute care for non-emergent conditions, chronic disease management, and Group Weight Management, with a limited cohort enrolling for 2026. Visits can be in person or by telehealth. In its own words: "We will help you reach your goals in our luxury outpatient clinic with nutrition and exercise counseling as well as medications." Its Google listing shows a 5 star rating from 9 reviews.
What does it cost?
Care is cash pay only. The practice does not bill insurance and charges a flat membership rate instead. The first-year weight management membership is $3,900 a year or $1000 a quarter. Primary care plus first-year weight management is $5000 a year or $1275 a quarter. After the first year, weight maintenance is $2,500 a year or $650 a quarter, or it is included in the primary care membership. It does not publish a standalone primary care price, so ask for one.
Every plan requires a $300 initial assessment, which is applied to the membership rate. The first-year plan is a 12 month commitment, and memberships may rise each year. It does not publish a cancellation policy. It does not say whether the membership price includes medication. Many members can use FSA or HSA funds for membership fees, depending on the plan. A membership does not replace health insurance.
What should you ask before you book?
Ask whether the membership includes medication. If it does not, ask what a prescription costs and where it is filled. The clinic does not name its weight loss medication, so ask which one it uses and whether it is brand-name or compounded. Ask which of its two physicians would prescribe for you and manage your follow-up.
Ask what testing costs beyond what the plan includes. The first year covers intake labs and an electrocardiogram. Ask how and when yearly price increases are set, and how to cancel during the 12 month commitment, including whether any part of the payment is refunded. Ask for office hours, which are not listed, and how long the practice has been open, which it does not say.
Who is it a good fit for?
This is a good fit if you want a flat-rate, cash-pay practice with physicians you can reach by phone, text or email, and you are ready to commit to a 12 month plan once the medication, renewal and cancellation terms are in writing. It may not suit you if you need care billed to insurance or expect the membership price to cover everything.
