About Perpetua Longevity
What is Perpetua Longevity?
Perpetua Longevity is a physician-led practice with one clinical office at 8905 West Post Road, Suite 110, in Las Vegas, Nevada. Its prescribers are Dr. Juan Chavez, MD, and Dr. Lucia Mireles-Chavez, MD, a husband and wife team. Dr. Chavez's credentials are Internal Medicine, Obesity Medicine, Clinical Lipidology, Anti-Aging & Regenerative Medicine, and Stanford Exercise Physiology. Dr. Mireles-Chavez's are Pediatrics, Obesity Medicine, Anti-Aging & Regenerative Medicine, and Aesthetics & Sexual Wellness. Together they report more than 40 years of combined clinical experience.
Its main business is a private, cash-pay concierge membership. Services include medical weight management with semaglutide and tirzepatide, hormone optimization, peptide therapy, cardiovascular prevention, diabetes and metabolic care, and aesthetic work such as Morpheus8. It also offers IV therapy, medical hair restoration, sexual and intimate wellness, and hormone and pellet therapy. Virtual care is open to eligible patients in Nevada and California, and patients from any state can travel to the Las Vegas office. Hours are by appointment only.
What does it cost?
The membership is $595 per month. That fee does not include prescriptions, medications, supplements, laboratory fees, DEXA scans, VO₂ Max testing, or imaging, so semaglutide or tirzepatide would be an added cost. A one-time $1,500 onboarding fee is listed with the membership. It does not say what that fee covers. The membership includes an initial consultation, quarterly physician consultations, quarterly lab review, and ongoing management of obesity, hormones, diabetes, hypertension, and cholesterol.
The practice does not bill insurance. In its own words: "No. Perpetua Longevity is a private concierge membership and does not bill insurance." Some separate charges may be eligible for reimbursement through insurance or an HSA or FSA. It does not publish a consultation fee, separate prices for weight loss or hormone care, or whether GLP-1 medication is included in weight-loss pricing. It also does not state its cancellation or renewal terms. Uninsured patients can request a Good Faith Estimate at any time by calling or emailing the office.
What should you ask before you book?
Ask what the $1,500 onboarding fee covers and whether any part of it is refundable. Ask what semaglutide or tirzepatide would cost each month on top of the $595, and whether those medications are brand-name or compounded. Ask who signs the prescriptions and how often you would see them.
In its own words: "You are entering a long-term physician partnership designed to protect your healthspan for decades." Ask how long the membership runs, how you cancel, and whether it renews automatically. Ask which peptides are used and what each one is for. Ask what quarterly lab work costs beyond the membership, and who bills for labs or imaging done at outside facilities. If you are considering pellet therapy or in-person care, ask what a Las Vegas visit involves and what it costs.
Who is it a good fit for?
This is a good fit if you can carry the $595 monthly fee plus medication and testing costs yourself, you live in Nevada or California or can travel to Las Vegas, and you want an ongoing relationship with physicians rather than a single visit. If you need one all-in price before you commit, get the answers to the questions above in writing first.
