About Top Form
What is Top Form?
Top Form is a medical clinic and wellness center in Papillion, Nebraska. It names Papillion and Omaha as its service area. Weight loss is the service it leads with. Its LoseIT program uses GLP-1 and GIP medications such as Wegovy, Saxenda, Ozempic, Zepbound and Mounjaro, and it also lists oral options like Contrave, Qsymia and phentermine. Men can get testosterone replacement, women can get bioidentical hormone therapy, and peptide therapy is offered as well.
The rest of the menu runs from IV hydration, Emsculpt and Emsella to chemical peels, lab draws, hair loss care and Quick Care telehealth for minor illness. No prescribing provider is named. Google reviews average 5 stars across 69 reviews. In its own words: "every service is designed to uncover the root cause of your symptoms, not just mask them."
What does it cost?
A new patient consultation costs $200. You pay a $50 deposit to start, and the remaining $150 is due when the consultation is complete. The monthly membership is $75 and does not include medication. It starts once a weight loss medication is prescribed or ordered, requires a three-month commitment, and needs 30 days' notice to cancel. Membership is required to keep getting care.
Labs must be done before the consultation. You can send them to a local lab, or draw them in the office for $125, which HSA or FSA funds may cover. Injectable medications run roughly $1000 to $1400 a month without insurance, and some drop to $650 with a savings card. Medicare and seniors 65 and older get 30% off the monthly and new patient fees, and first responders and military get 20% off. Those discounts do not apply to medication or labs. The clinic does not publish prices for hormone therapy or peptides.
What should you ask before you book?
Ask who writes the prescription and what their name and license are, since none is published. Ask whether the medication is brand-name or compounded, because the weight loss material includes a section on compounded options. Ask what the drug will cost each month once prescribed, and whether your insurance covers it.
Ask what the follow-up visits, due every three months, and the annual review cost, plus any labs ordered after the consultation. Ask for a quote on hormone therapy or peptides before you start. Ask which of the two Papillion street addresses the clinic lists your visit will be at, and what its hours are, since neither is published. Finally, ask whether the three months are billed up front, and what happens to the $50 deposit if you miss an appointment without 24 hours notice. That deposit is nonrefundable.
Who is it a good fit for?
This is a good fit if you want weight loss or hormone care you pay for in cash, you can commit to three months of membership, and you are ready to pay for medication on top of the monthly fee. If you want hormone or peptide care, get a quote first, since those prices are not posted.
