About FORGED WELLNESS & CO.
What is Forged Wellness & Co.?
Forged Wellness & Co. is a proactive longevity medicine practice at 3300 Raser Dr. in Missoula, Montana. It is open by appointment only. It offers hormone therapy (HRT and TRT), GLP-1 medication for metabolic health and weight management, peptide therapy and longevity medicine, plus metabolic, hormone and gut health programs, advanced testing, vitamin shots and IV therapy, and workplace and executive partnerships. A 1:1 telehealth visit with a clinician is offered.
It does not name a prescribing physician. Its team is listed as board certified medical practitioners, registered nurses, a registered dietitian, a health coach and a personal trainer. The medications it names are sermorelin and GLP-1 receptor agonists. Where clinically appropriate, it prescribes compounded medications from 503A and 503B pharmacies. In its own words: "We do not provide primary care or urgent care services for members."
What does it cost?
The initial visit is $399. Programs are billed monthly, but it does not publish its monthly fees. Membership prices do not include medication, and it does not publish hormone or GLP-1 medication prices. Hormone medications, labs and specialty testing may be billed separately.
Billing works two ways. In Hybrid Care, medical visits are billed to insurance when they are medically necessary and eligible, and non-covered services are billed at self-pay rates. In Concierge Care, provider visits are included in the monthly fee and are not billed to insurance. It is in network with select insurance plans, but it does not accept Medicare, Medicaid, Humana or UnitedHealthcare, and it does not say which plans it does accept.
The hormone program has a 3-month minimum. In its own words: "After the initial 3 months, the program moves to month-to-month with 30 days written notice to cancel or transition."
What should you ask before you book?
Ask who signs the prescription, since no prescriber is named. Ask what hormone or GLP-1 medication will cost each month, because the membership does not include it. Ask whether each medication is brand-name or compounded. Ask which peptides are available today, because it will not offer the peptides now under FDA review until that review is finished. Ask what follow-up labs and visits cost. Ask which insurance plans are in network. It does not publish a phone number, so ask how to reach the clinic, and ask how cancellation works after the first three months.
Who is it a good fit for?
This is a good fit if you want an ongoing, lab-based program in Missoula, expect to pay mostly out of pocket, and can accept that medication sits outside the membership fee. It is a poor fit if you need primary or urgent care, since it does not provide either. Get the answers to the questions above before you book.
