About Revive Body Optimization

What is Revive Body Optimization?

Revive Body Optimization is a hormone and weight loss practice at 1310 9th Ave S in Great Falls, Montana. It was founded by two brothers, Chad Sievers and Eric Sievers, who grew up in the area and prescribe there.

Its services fall into four groups: hormone services, weight loss, sexual wellness and hair loss. Testosterone therapy comes as an injection, a topical or an oral option, and there is a version for women. The menu also includes nandrolone, HCG and oxandrolone. The weight loss services use semaglutide, tirzepatide (including an oral form) and phentermine, and sermorelin appears under body optimization. A wellness consultation covers options the practice does not specifically offer.

Visits can happen by video. Booking starts with a new patient form, then an OnPatient portal account. In its own words: "All appointments are booked directly inside the portal." It has a Google rating of 5 from 3 reviews.

What does it cost?

The consultation is free, and prices are set per program rather than per visit. Semaglutide is $725 for the initial program, $825 for low dose maintenance and $925 for high dose maintenance. Tirzepatide is $850 for the initial program, $1,075 for low dose maintenance and $1,275 for high dose maintenance, and the oral form starts at $500. Phentermine is $395.

Testosterone is $425 by injection, $475 topical and $375 oral, and testosterone for women starts at $425. Sermorelin and nandrolone are $625 each, HCG with testosterone is $595, and oxandrolone is $375 for men or women. Post cycle treatment is $120, sexual wellness for men is $75, hair loss treatment is $180 and a wellness consultation is $150.

It does not say whether the price includes medication. It does not publish insurance information. The testosterone program runs in 12 week cycles, and a refill needs another lab evaluation and consultation.

What should you ask before you book?

Does the program price include the medication? If not, what does the medication cost each month?

Is the semaglutide or tirzepatide brand name or compounded? Ask for the name of the product and its source before you pay.

Who writes each prescription, and how often are follow-up visits? What do follow-up visits and lab work cost, given that labs must be from the last 12 months?

How do you cancel a program, and is any part of a paid program refundable? Cancellation terms are not published.

Who is it a good fit for?

This is a good fit if you want a video-based hormone or weight loss program in Great Falls and can pay the program price plus any medication cost yourself. It is worth calling if you want the prescribing brothers to answer the questions above before you commit.
