What does it cost?

It does not bill insurance. In its own words: "While Crafted does not directly bill/accept insurance, we are happy to provide you with visit documentation you may need to submit to your insurance company." Expect to pay cash and submit the paperwork yourself.
What should you ask before you book?
Ask who writes the prescription. It does not say which provider prescribes GLP-1 medications or hormones. The two people it lists as prescribers are Alaina and Fallon Melby, RN, so ask which of them you would see and whether that person is at the office you choose.
Ask whether the monthly fee includes the medication, and whether the semaglutide and hormones are brand-name or compounded. Ask what follow-up visits and lab work cost, whether telehealth is available (it does not say), and how cancellation works, since it does not state cancellation terms. Ask for the consultation fee before you book.
It lists eligibility criteria, including being 18 or older, and says some people need special medical clearance, such as people with diabetes, who must be cleared by an endocrinologist. Ask which of these apply to you before you commit.
Who is it a good fit for?
This is a good fit if you want cosmetic treatments and medical weight loss or hormone care from one clinic, you are prepared to pay cash and file insurance paperwork yourself, and you are comfortable getting the prices, prescriber and cancellation terms in writing before you start.
