MaineCHI is an integrative clinic with offices in Winslow and Waterville, Maine. Dr. Kathryn Mari Sawai, ND, and her husband, Isaac, co-founded it in 2015. She is the one prescriber the clinic names.

What does it cost?
The Functional Weight Loss Program costs $450. That covers a screening visit, a fasting blood draw, a lab review and plan visit, the first month of tirzepatide injections at no charge, a syringe carrying case, and three 15-minute follow-up visits.
The clinic does not say whether tirzepatide is included after that first month, and it does not publish a price for the injections that follow. Add-ons such as NAD+ and CJC/Ipamorelin or Sermorelin are billed separately, and no prices are published for them or for peptide therapy. It publishes no consultation fee, membership terms or cancellation terms. It takes insurance. Its program materials say the fasting blood draw can be billed to insurance or paid through the Vibrant America self-pay option for a full functional profile.
What should you ask before you book?
Ask who writes and signs the tirzepatide prescription. Samantha Pelkey, RN, is named as your dedicated clinician, but nothing shows she prescribes.
Ask which applies to you, because the clinic's peptide materials say metabolic peptides require established patient status and are not offered to new patients at the first visit. Its program description places the first tirzepatide injection at the lab review visit.
Ask whether the $450 covers medication after the first month, and what that medication costs. Ask whether the tirzepatide is brand-name or compounded. Its peptide materials describe compounding pharmacy sourcing but do not say which applies to tirzepatide.
Ask what the consultation, follow-up visits and repeat lab work cost once the program ends. Ask how to cancel and whether anything renews automatically.
Who is it a good fit for?
This is a good fit if you want weight loss care and peptide options at a clinic that also offers chiropractic, acupuncture and massage. It is also a fit if you are willing to get the medication price, the prescriber and the cancellation terms in writing before you commit. If you need every fee published online before you call, expect to ask more questions first.
