What is Live Life Family Practice?
Live Life Family Practice is mainly a direct primary care practice. Members pay a fee instead of billing insurance, and the clinic also offers medical aesthetics and wellness treatments. Its medically supervised weight loss program is one of those wellness services, alongside IV hydration, laser hair removal, body contouring and outpatient lab work.
Cortney Muldoon, FNP, opened the practice in South Paris, Maine, in 2021. It now has offices in Poland and South Paris, and the Poland office opened in 2025. The clinicians it names are Cortney Muldoon, FNP, Rachael Baillargeon, FNP-C, Katie Sturtevant, RN, and Jordyn Johnson, RN. None of them is listed as a physician. The weight loss program uses semaglutide, which the clinic also calls Ozempic, and it is offered at both locations. Google shows a 4.8 rating from 16 reviews.
What does it cost?
Membership is $100 a month on a 6-month minimum contract, or $1,100 a year if paid in full at enrollment. It covers primary care visits, wholesale labs and medications, and some minor in-office procedures. Non-members pay $300 or more for a walk-in or acute visit. Fees are collected on the 15th of each month from a bank account on file. The clinic does not accept insurance, though it recommends a low-cost, high-deductible plan for emergencies. Cash and check are also accepted, and Medicare patients are welcome under a one-time waiver.
The weight loss program is not part of the membership. It is billed separately, and members receive a discount on in-house labs and pharmaceuticals. The clinic does not publish weight loss prices. In its own words: "Please call for pricing. The cost varies based on your treatment plan and goals." It does not say whether medication is included, and it lists no consultation fee. Missed appointments without 24 hours notice can cost $50, or $75 for visits that include blood work, after two missed visits.
What should you ask before you book?
Ask who writes the semaglutide prescription and whether a physician is involved, since none of the clinicians named is one. Ask whether the medication is brand-name Ozempic or a compounded version, and whether the program price includes it. Ask what the first consultation, follow-up visits and lab work cost, and whether you need a membership to join the program.
On cancellation, the clinic's wording is: "After your initial 6 months, there are no long-term contracts in place. You may cancel anytime, and services will continue through the end of your current billing cycle." Still, ask whether the $150 re-enrollment fee applies if you stop only the weight loss care. Also ask what happens after late payments, since three missed or late payments end membership.
Who is it a good fit for?
This is a good fit if you want a cash-pay primary care membership and are comfortable paying out of pocket for weight loss care without a published price. If you want a fixed price or a physician as the named prescriber, get those answers before you book.
