About Empower Wellness Center
What is Empower Wellness Center?
Empower Wellness Center is a family practice at 12722 Tonkel Road in Fort Wayne, Indiana. Its phone number is (260) 284-4880. Stacie Housholder and Jennifer Mays are Family Nurse Practitioners and co-owners. Dustin Diller is a Family Nurse Practitioner, and Kori Engdahl holds the credentials CNM WHNP-BC. Those four are the prescribers the clinic names.
Primary care, acute care and physicals make up most of the work, including D.O.T., sports, and general work or camp physicals. Two of the services people look for in cash-pay care also appear here. Weight management is part of the membership, and hormone replacement and testosterone management are named under Functional Health. In its own words: "With the membership, patients have access to unlimited primary care (including weight management and hormone therapy) and acute care visits." The clinic does not name peptide therapy or longevity medicine. It uses a compound pharmacy for specific patient medication needs. Google shows a 5 rating from 6 reviews.
What does it cost?
The adult membership is $100 per month, with a one time $10.00 initiation fee. Children are $30.00 per visit with an active adult parent membership. Re-initiation costs $200. Memberships can be cancelled at any time with 30-day notice. The clinic does not say whether the monthly fee includes medication, and it does not list a separate price for hormone therapy.
Outside the membership, primary care and acute care visits start at $110. A Functional Health initial visit (90 minutes) is $200, and a follow-up (60 minutes) is $150. Labs are not included. D.O.T. physicals are $100, and sports and general work or camp physicals are $40 each. Travel Health is $49 and does not include medications or vaccines. It accepts most major insurance providers.
What should you ask before you book?
Ask whether the $100 monthly fee covers any medication, or whether prescriptions are billed on top. Ask which medications are used for weight management, and whether they are brand name or compounded, since the clinic uses a compound pharmacy. Ask which prescriber would sign your prescription and how often you would see them. Ask what follow-up visits and lab work cost, because labs are not part of the Functional Health price. The clinic does not say whether it offers telehealth or what its hours are, so ask both. If you plan to use insurance, ask whether any of your visits would be billed to it or whether the membership stays cash only.
Who is it a good fit for?
This is a good fit if you want an ongoing primary care membership in Fort Wayne and weight management or hormone therapy would be part of that care. It is a weaker match if you are looking for peptide therapy or longevity medicine, since the clinic does not name either.
