What is Bloom Hormone Health?
Bloom Hormone Health is a cash pay hormone clinic with two offices, both in Fort Wayne, Indiana. Its main focus is hormone management for men and women. What does it cost?
Prices are per visit. A hormone consult is $308 for one hour, and its page shows a cash discount price of $296. A Follow Up Consultation is $156 for 30 minutes and covers follow up lab review and medication management. A Pellet Insertion Appointment starts at $301 for 30 minutes. The clinic does not say whether medication is included in any of these prices.
It does not publish prices for weight management, thyroid management, or peptide therapy. It does not accept insurance, but it will provide a superbill you can submit to your insurer for possible reimbursement. Labs are usually drawn at an outside lab, which may be covered by insurance, and the clinic offers limited onsite lab work on most Fridays for registered patients. To cancel or reschedule, contact the clinic 24 hours before your appointment to avoid a cancellation fee. It does not mention financing or HSA payment.
What should you ask before you book?
Start with who signs the prescription. Ask whether Dr. Bauer will be involved in your care or only the nurse practitioner you see. Ask whether the $301 pellet price or the $156 follow up includes the hormone or medication itself, and whether any GLP's or other peptide is brand name or compounded, and what it costs each month. Ask for prices on weight management and peptide therapy before you book, since neither is published. Ask how often follow ups are required and what labs cost out of pocket. The clinic does not say whether telehealth visits are offered, so ask about that too.
The peptide service has its own condition. In its own words: "This service is for established patients only and requires regular follow up appointments." If you are new, ask whether you must wait before starting it. Calling 260-999-7147 is the simplest way to get these answers before you pay.
Who is it a good fit for?
This is a good fit if you live in or near Fort Wayne, are ready to pay cash, and want a practice where hormone care is the main focus. It is less likely to suit you if you need a clinic that takes insurance, or a longevity or anti aging program, which the clinic does not offer.
