What is Piatek Institute?
Piatek Institute is a medical weight loss practice at 13421 Old Meridian St, Suite 200, Carmel, IN 46032. It has one office, in Carmel, and has been serving central Indiana since 1995. Dr. Jay Piatek is the prescriber the practice names. No degree is listed after his name. What does it cost?
The compounded program starts with a $399 package. It covers your consultation with Dr. Piatek, an EKG, a basal metabolic rate test, labs, your first vial and four months of appointments. After that, compounded semaglutide is $179 a vial and compounded tirzepatide is $249 a vial, the same at every dose. Shipping is $25, or $35 expedited, and pickup in Carmel is free. Visits after month four are $135 each, or from $75 a visit on the year plan. There are no contracts or subscriptions, and vials are billed when you refill. The compounded program is cash pay with no insurance billing, and HSA and FSA cards are accepted.
On the Medicare program, the $50 monthly medication copay is paid at the pharmacy and does not go to the practice. The practice's own charges are visits, at $570 for six months or $900 for twelve, and a single visit is $135. It also charges $50 each time a prior authorization is submitted. Phentermine starts around $159 a month. The practice does not publish prices for hormone therapy or for brand-name GLP-1 medication.
What should you ask before you book?
Ask what credentials Dr. Piatek holds, since none are listed, and who writes and manages your prescription after the first visit. Ask whether the $399 start includes the medication you will use, and whether the per-vial price holds at every dose. Ask which visit price applies after month four, $135 or $75, and what follow-up lab work costs, since neither is published.
Ask whether compounded medication is the right route for you. The practice's own disclaimer says compounded medications are not FDA-approved products. Ask what hormone therapy costs, because no price is listed. Finally, ask how you would cancel. The practice says there are no contracts, but it does not describe the steps to end care.
Who is it a good fit for?
This is a good fit if you want GLP-1 medication prescribed in person in Carmel, you are ready to pay cash, or you are a Medicare patient who wants to check eligibility first. It is not the right place for peptide or longevity medicine, which the practice does not offer.
