What is CorePoint Health and Medicine?
CorePoint Health and Medicine is a functional medicine and aesthetics practice at 3965 West 106th Street, Suite 140, in Carmel, Indiana. It was known as Integrative Medicine, Laser and Aesthetics before the name change. Its main work is functional medicine and aesthetics, with IV infusions, urgent care, telemedicine and a membership model alongside. Weight management and hormone optimization are also among its services, next to Morpheus8, microneedling, laser hair removal and skin care. It says it serves Carmel, Zionsville, Indianapolis and the surrounding areas. Google shows the practice at 4.9 from 102 reviews.
The only prescriber the clinic names is Dr. CJ Pabla, MD. Dr. Jason Amich, DHSc, MBA is named as a provider as well, but the clinic does not say which of them treats weight loss patients. Its weight loss material says the clinic helps people decide between semaglutide and tirzepatide, and it names Ozempic and Wegovy too.
What does it cost?
It does not publish prices, and it gives no consultation fee. The clinic bills by membership instead of insurance. In its own words: "Rather than billing insurance, patients are billed by an affordable annual or monthly fee." Three membership levels are named, Basic, Gold and Platinum. Basic includes initial functional medicine visits, and Platinum adds specialty labs and 24/7 phone access to your physician. The clinic does not say whether medication is included in any fee, and it does not explain how cancelling works.
What should you ask before you book?
Ask who signs the prescription for semaglutide or tirzepatide, and whether that is Dr. Pabla, Dr. Amich or someone else. Ask whether the monthly or annual fee includes the medication, and whether the drug is brand-name or compounded. Ask what follow-up visits and lab work cost, and whether they fall inside a membership. Ask whether the 90 to 120 minute initial visit is included at the Basic level, and whether weight loss care can be done by telemedicine. Ask whether hormone replacement such as HRT or TRT is prescribed, since the clinic does not say. Finally, ask about the membership commitment, how to cancel, and office hours, which are not listed.
Who is it a good fit for?
This is a good fit if you want a membership-style practice in Carmel, are ready to pay out of pocket, and want to talk through semaglutide or tirzepatide with a physician before you decide. It is a weaker fit if you need published prices, a clear answer on what the fee covers, or peptide therapy or longevity medicine as their own services, which it does not list.

We have not yet collected published prices or prescriber credentials for this clinic. When we do, they will appear here with a link to the page they came from — or a note saying the clinic publishes none, which is itself worth knowing. Call them for a price in the meantime.
