About Tate Integrated Wellness

What is Tate Integrated Wellness?

Tate Integrated Wellness is a direct primary care practice at 204 Pinecrest Dr in East Peoria, Illinois. Lindsay Tate, MD, is the prescriber, and Dr. Tate is board certified in family medicine. It works on a monthly membership and does not bill insurance companies. Its listed services are IV therapy, basic BOTOX® and dermal fillers, hormone balance for women, weight loss, preventative medicine and chronic disease management. It also has a men's health section that covers testosterone. The office is open 9 AM to 5 PM, Monday through Friday, and closed on weekends. In its own words: "Our Direct Primary Care model means we provide healthcare for a straightforward membership fee." It has a Google rating of 5 from 8 reviews.

What does it cost?

Membership pricing runs from $50 a month for children 5 to 8 and college students with a valid student ID, up to $200 a month for a family of up to two adults and two children ages 5 to 18. Adults pay $75 a month from ages 19 to 40, $100 from 41 to 54 and $110 from 55 up. A couple pays $175 a month. Non-members pay $75 for an acute care visit and $200 for a one-time full physical. A single IV is $185, two IVs are $170 each and three or more are $125 each.

The women's hormone program lists a membership plan at $60 a month. Its description says the membership includes custom-compounded medications, but it does not name which hormones are covered. Labs are a separate cost. In its own words: "Labs are mandatory after the initial consultation. Additional fees apply."

Weight loss is listed among the services the membership covers, but it names no weight loss medication and gives no price for a weight loss program. It does not publish prices for BOTOX or filler visits, and the membership does not include them. It does not publish men's hormone pricing, a consultation fee, or membership terms and cancellation policy.

What should you ask before you book?

Ask whether the $60 hormone membership covers the hormones themselves, and which ones. Ask whether those medications are custom-compounded or brand-name, and who writes and adjusts the prescription. Ask what the initial lab panel costs, and what the follow-up labs at 3 and 6 months cost, since the program requires them.

If weight loss is your goal, ask which medications the program uses and what it costs, because neither is published. Ask whether follow-up visits can be done by video or text, since telehealth is not stated. If you are a man asking about testosterone, ask for the price before booking.

Finally, ask how the membership is cancelled, how much notice is required, and whether prepaid months are refunded. None of that is published.

Who is it a good fit for?

This is a good fit if you want a physician you can reach directly for primary care, you are comfortable with a flat monthly fee in place of insurance billing, and you will call before committing to the hormone, weight loss or cosmetic programs, since their prices and terms are not all published. The practice itself recommends keeping insurance for specialty care, emergencies and hospital stays. Call 309-605-0474 with your questions.
