About Spa 35 Medical Spa

What is Spa 35 Medical Spa?

Spa 35 Medical Spa is a medical spa at 403 S. 11th Street, Suite 135, in downtown Boise, Idaho. It has been open since 2006. Most of what it offers is cosmetic: Botox and Dysport, lip filler, laser hair removal, CoolSculpting, microneedling, age spot and skin tone correction, and tattoo removal. Its medical weight loss and hormone replacement therapy sit alongside those services.

The weight loss program uses GLP-1 peptide injections, and the clinic names semaglutide and tirzepatide. Every patient starts with blood work and a consultation with a Nurse Practitioner. A Registered Nurse gives the injections in the office on its month-to-month plan. The clinic does not name the prescriber.

In its own words: "Every patient begins with blood work and laboratory analysis so your Nurse Practitioner can calibrate your protocol precisely to your body's needs."

The office is open Monday and Wednesday 9am to 4:30pm, Tuesday and Thursday 9am to 5:30pm, and Friday 9am to 4pm. Weekend hours are not listed. The clinic has a 4.9 rating on Google from 753 reviews.

What does it cost?

The GLP-1 program starts with a $49 initiation fee, which covers blood work, lab analysis and the Nurse Practitioner consultation. After that, there are two ways to pay. The month-to-month plan is $599 per month, with weekly in-office injections and no long-term commitment. The six-month membership is $319 per month in Phase 1, $399 in Phase 2 and $479 in Phase 3. It includes auto-pen home delivery from a compounding pharmacy, and it saves 20% compared with month-to-month. Membership is auto-billed monthly, and a $500 early termination fee applies if the agreement is not completed.

The clinic does not say whether the medication is included in the monthly price. It does not say whether it accepts insurance either. Payment plans and financing options may be available. Many of its other treatments start at a base price, with final pricing set at a free consultation.

What should you ask before you book?

Ask who signs the GLP-1 prescription and what their license is. The clinic does not name that person.

Ask whether the medication is included in the $599 or the membership price, or billed on top of it. Ask whether it is brand-name or compounded, since the membership ships through a compounding pharmacy.

Ask what follow-up lab work and visits cost after the first month.

Ask whether HSA or FSA cards are accepted and whether any part of the care is billed to insurance.

Ask whether any visits can be done by video. The clinic does not say.

Ask how you cancel a membership, what counts as completing the agreement, and whether the $500 fee applies if you stop early. Ask for the total you would pay over six months in writing before you pay the $49.

Who is it a good fit for?

This is a good fit if you live in or near Boise, Meridian or Eagle, want in-person care, and can pay out of pocket each month with the cancellation fee in mind. If you need a named prescriber or insurance billing, get those answers before you book.
