About Vitality Functional Medicine

What is Vitality Functional Medicine?

Vitality Functional Medicine is a functional medicine practice at 130 Professional Drive, Unit B, in Ponte Vedra Beach, FL 32082. Its main work is hormone, thyroid and metabolic care, along with integrative primary care. Two prescribers are named: Kelley Solosky, APRN, FNP, who leads the hormone, thyroid and weight care, and Dr. Adam Perlman, MD, MPH, who handles the functional medicine and concierge primary care.

The clinic lists seven services: functional integrative medicine and primary care, hormone optimization for women and men (including bioidentical hormone therapy and testosterone therapy), thyroid and metabolic health, medical weight loss, medical acupuncture, wellness and longevity medicine, and specialty consultation. Peptide therapy is not among them. The hormone membership includes optional monthly virtual check-ins between quarterly visits. In its own words: "Healthcare should not be rushed, reactive, or fragmented."

What does it cost?

The clinic does not publish one clear price list, and its prices disagree from page to page. The initial consultation appears as $299 in one place and $275 in others. The hormone membership is shown as starting at $135 a month, the ongoing care membership is $150 a month, and integrative primary care membership is $199 a month.

Standalone medical weight management is $99 a month plus medication costs, and the medication is not included. Hormone and primary care members get weight management at no added charge when it is clinically appropriate. Compounded all-inclusive hormone plans start at $225 a month, with prescribed medication shipped to your home. A follow-up visit for someone not enrolled in a membership is $190. The concierge primary care option lists a $750 initial consultation and $375 follow-up visits.

Membership is month-to-month and can be canceled at any time with 30 days' notice. Medications and lab testing may be bought separately or billed through insurance when applicable. The clinic does not say whether office visits are billed to insurance.

What should you ask before you book?

Ask which first-visit price and which monthly price you will be charged, and get the answer in writing. Ask whether the monthly fee covers any medication, since weight loss medication is billed separately. Ask which weight loss medications the clinic uses, because none are named publicly, and whether they are brand-name or compounded. Ask what follow-up visits and lab work cost, and whether your insurance will be billed for any of it. Ask how many visits and check-ins are included each quarter, and who will sign your prescriptions. Ask how a cancellation is submitted, and whether the 30 days' notice must be in writing. No office hours are listed, so ask for them too.

Who is it a good fit for?

This is a good fit if you want ongoing hormone, thyroid or weight care from a prescriber who follows you between visits, and you can cover the monthly fee plus medication and lab costs. If you need one firm price before booking, call 904.560.5815 first, since the clinic's published prices do not match.
