About Palm Coast Family Practice
What is Palm Coast Family Practice?
Palm Coast Family Practice is a family practice at 9 Pine Cone Dr. Ste. 102, Palm Coast, FL 32137, and it has practiced family medicine in Palm Coast since 2000. Primary care, urgent care and walk-in visits are its main work. Weight loss is one of several services, alongside immunizations, EKG testing, on-site X-rays, sports, school and D.O.T. physicals, and same-day telemedicine visits for sickness or prescription refills.
Matthew Dorman, MD, is the physician. Pablo De Jesus, PA-C, and Humberto Encalada, PA-C, are physician assistants. Kelley Wise, APRN, Chelsey Lossen, APRN, Abigayil Anderson, APRN, Skye Sauls, APRN, and Augusta Slocum, DNP, are nurse practitioners.
The weight loss program is GLP-1 based and names two medications, tirzepatide and semaglutide. In its own words: "our doctor-supervised weight loss program combines BMI and BFP analysis with personalized care." The practice's Google listing shows a 2.8 rating from 13 reviews.
What does it cost?
The clinic publishes self-pay visit prices of $200 for new patients and $175 for established patients. It does not publish a price for the weight loss program or a weight loss consultation fee, and it does not say whether medication is included.
The direct primary care membership is $100 a month plus a $175 one-time enrollment fee. Services outside the membership are typically around 30% off retail, covering labs, X-rays and in-office procedures. An established self-pay patient who joins the same day can have that $175 visit count toward the enrollment fee. In its own words: "No long-term contracts. Membership is month-to-month. You can cancel any time."
Most major insurance plans are accepted, including Aetna, BCBS, Cigna, Florida Blue, Florida Healthcare Plans, Medicare, Oscar and Tricare. Many members pay with HSA funds, but confirm eligibility with your HSA administrator first.
What should you ask before you book?
Ask which provider prescribes the medication. Weight management is among Dr. Dorman's areas, but it does not name who runs the weight loss program. Ask whether the membership or any program fee includes the medication, whether the drug is brand-name or compounded, and whether your insurance covers it at all.
Ask what follow-up visits and lab work cost, and where prescriptions are filled and at what price. Ask whether telehealth is available for the program and in which states, since it does not say. Finally, ask how screening works for the conditions the clinic lists as ruling out the medication, and how to cancel the program itself.
Who is it a good fit for?
This is a good fit if you want a family practice that handles everyday illness, urgent care, and sports or D.O.T. physicals along with a weight loss program, and you are willing to get the medication, program and screening details in writing before you pay. It is a weaker fit if you need a published price for the weight loss program before you call.
