What is Awaken Health DPC?

Awaken Health DPC is a membership-based direct primary care practice at 10400 Griffin Rd, Suite 204 in Cooper City. What does it cost?

The practice runs on membership: a one-time $99 enrollment fee and a 6 month commitment, then $199 a month for Tier 1 or $249 a month for Tier 2, for ages 14 and up. Pricing is at the doctor's discretion after your first consultation. Weight loss medications come at an additional charge, and members get wholesale medication pricing. It doesn't bill insurance for office visits.

After the first 6 months you can cancel at any time with no penalty. Monthly payments are non-refundable and can't be paused, and rejoining later costs a $199 re-registration fee. The weight loss page offers a free consultation, and the first meet and greet with the doctors is a free video call.

What should you ask before you book?

Ask whether the weight loss program is included in the monthly membership or billed on top of it. Ask which GLP-1 medication you'd be prescribed, whether it's brand-name or compounded, and what it costs each month, since the medication is extra. Ask which doctor would manage your care and sign the prescription. Ask what the intake lab work and any later labs cost. Ask whether check-ins can be done by video. Leaving the membership ends prescriptions and refills from the practice, so ask how a weight loss medication would be handled if you cancel.

Who is it a good fit for?

This is a good fit if you want a primary care doctor and a supervised weight loss program in one place, and you're comfortable with a monthly membership and a 6 month commitment. It's less of a fit if you only want a GLP-1 prescription without joining a primary care practice.
