What is PUR-FORM?
PUR-FORM is a cash-pay clinic at 3600 FAU Blvd, Ste 101, in Boca Raton, Florida. Much of its menu is regenerative and wellness care, from stem cell therapy and platelet rich plasma to hyperbaric oxygen, cryotherapy and vitamin IVs. Its weight loss, hormone and peptide services sit alongside NAD+ and erectile dysfunction treatment. Semaglutide is the one medication it names.
Dr. Joseph Purita, MD, is the one prescriber the clinic names. It lists him as medical director and says he is board certified in orthopedic surgery, pain management and regenerative medicine. Google lists the clinic at 4.8 from 249 reviews.
What does it cost?
It does not publish prices for medical weight loss, hormone therapy or peptides. The prices it does show are for memberships. Pur-Wellness is $159 a month and includes 5 monthly wellness treatments and one level-1 vitamin shot. Pur-Lifestyle is $269 a month and adds more monthly treatments and a level-1 IV. Pur-Elite is $529 a month and includes 30 monthly wellness treatments plus boutique fitness center access.
The weight loss page offers 3 free months of membership with its medical weight loss packages, but it gives no package price. The first consultation is free for non-members and runs 30 minutes. Its only named payment option is financing through Cherry. It does not say whether it accepts insurance, whether medication is included in any price, or what the cancellation terms are.
What should you ask before you book?
Ask who writes and signs your prescription, and whether you will see Dr. Purita in person. Ask whether the monthly fee includes medication. If it does not, ask the price of each drug and whether it is brand name or compounded. Ask what follow-up visits and lab work cost, since the assessment includes hormone testing and metabolic analysis. Ask whether telehealth visits are available, and how to cancel a membership, including whether there is a minimum commitment.
The fine print says PUR-FORM does not claim its regenerative applications are approved by the FDA, or even effective. Ask which approvals apply to the treatment you are considering. Finally, confirm the Saturday hours before you plan a visit, since the site shows both 10:00am to 5:00pm and 10:00 to 14:00.
Who is it a good fit for?
This is a good fit if you want weight loss, hormone care and wellness treatments in one Boca Raton office, and you are comfortable paying cash and getting prices in writing before you book. If you need published prices or insurance billing, call first and get those answers before you commit.
