About Revolutionary Wellness and Weight Loss
What is this clinic?
This is a cash-pay practice with offices in Vernon and Middletown, Connecticut. Its main work is medication-based weight loss. Testosterone replacement and men's health care make up the other large part of its services. It also lists peptide therapy and anti-aging care among its specialties but gives no detail on either.
Jena is the only prescriber named, described as a Doctor in Nursing Practice. Other staff are called "professionally trained providers," but no other prescriber is named. The first visit is virtual, through a telemedicine platform, and compounded medication is shipped to your home. The medications named include semaglutide and tirzepatide, each with B12, along with brand-name Ozempic, Wegovy, Mounjaro and Zepbound. Other options are phentermine with or without topiramate, bupropion with or without naltrexone, Contrave, metformin, acarbose and Vyvanse. For men's health, it names testosterone replacement, sildenafil and tadalafil. Its Google listing shows a rating of 5 from 5 reviews. It does not list hours or how long it has been open.
This listing is for its Middletown office at 386 Main Street, Fl 4, Suite 415.
What does it cost?
Weight loss starts at $315 a month, and testosterone replacement starts at $150 a month. The first visit is $450 for the consultation and the first month of GLP-1 medication, then $315 a month. A first visit for Contrave or phentermine is $300, then $150 a month. A plan with a pharmacy prescription starts at $150, then $75 a month. None of these prices apply to active CT Medicaid patients. The subscription list prices semaglutide/B12 at $315 a month for all doses, and says pricing may change.
You pay a $50 deposit at first contact for the consultation. If you decide not to join a subscription plan, a $150 consultation fee is nonrefundable. A 25% new-patient discount on consultations is advertised, but you have to inquire about it. The clinic accepts CT Medicaid and cash pay, and it will provide a superbill for your insurer without guaranteeing reimbursement. Membership requires 30 days' notice to cancel, and no refunds are given.
What should you ask before you book?
Ask who signs your prescription and whether anyone besides Jena does. Ask whether the $315 monthly fee covers the medication itself. Ask whether your semaglutide or tirzepatide is compounded or brand-name, since the subscription list describes semaglutide/B12 as a compounded prescription. Ask what follow-up visits and lab work cost. Labs are ordered through the practice for cash or through Quest for insurance, and no lab prices are published. Ask which company name will appear on your contract, since its fine print uses several. Ask which towns it serves and which states its telehealth care covers, because neither is stated. Ask how cancellation works, in writing, before you pay the consultation fee.
Who is it a good fit for?
This is a good fit if you pay cash, you are comfortable starting with a virtual visit, and you want medication-based weight loss or testosterone care from a prescriber you can confirm by name. It is a poor match if you need insurance to carry most of the cost, since it takes only CT Medicaid and cash pay.
