## What does the practice offer?

The semaglutide page describes the drug as "a cutting-edge treatment approved by the FDA for chronic weight management and Type 2 diabetes" and gives "Ozempic" as the common brand name. Other services appear in the site's list, including medical weight loss, injectables, complete blood work, physical examinations and Galleri early cancer detection, but no page we opened describes them, so we are not presenting them as confirmed. The clinic is open 8:30 AM to 5:00 PM Monday through Friday.
## What does the membership cost, and what comes with it?
The published figure is "$250 monthly*", with the asterisk as printed. Two conditions travel with it. There is a six-month minimum commitment, and a twelve-month commitment holds the membership rate for three years. The one inclusion recorded in full is two complimentary IV therapy treatments a year, which the page values at $578. Further inclusions are published on the membership page and are worth reading there directly: priority scheduling, a nutrition and wellness plan, vitamin B injections, an annual wellness exam, physician access by message, longer visits, same-day prescriptions and specialist referrals. Quoting the $250 without the six-month minimum would misrepresent the offer.
## What else might you pay?
Self-pay patients who are responsible for their own bills are required to pay the consultation fee at the time of service. The size of that fee is never stated. Cancelling with less than 24 hours notice, or not showing up, costs $50; the policies page prints that rule as "Failure to cancel or or no-show," doubled word and all. No price exists anywhere for the semaglutide program, per month, per dose or starting-at.
## Does it take insurance, and does it do telehealth?
This is not a pure cash-pay clinic. The policies page displays a set of accepted plans as badges, and there is a separate page in the navigation for Medicare patients. Patients with a co-pay under an insurance agreement are required to pay before meeting the provider. Read the payer list on the policies page rather than relying on a summary of it. Telemedicine is offered, and the page claims patients "can reach us regardless of their location worldwide." That is a claim about reachability, not about licensure, and no state is named, so it should not be read as nationwide care.
## Who would you see?
Two people are named: Ali Sheybani, MD, who has his own bio page, and Dr. Sheila Mahdaviani, who appears on the semaglutide page with no role, credential or bio page published.
