What is San Diego Metabolic Health & Direct Primary Care?

San Diego Metabolic Health & Direct Primary Care is a cash pay membership practice at 280 Riverview Parkway, Suite 101, in Santee, California. Its main work is medical weight loss and metabolic care. It also offers direct primary care, bioidentical hormone replacement therapy (BHRT) and body composition analysis, done on site. Hours are Monday through Thursday, 9:00 AM to 4:30 PM, and Friday, 9:00 AM to 3:00 PM.

Brian Lenzkes, MD, is board certified in internal medicine and serves as medical director. Leah Koskinen, PA-C, is a board certified physician assistant and is the provider named for BHRT. Those two are the only people listed as prescribers. Mary DePuy, office manager and health coach, and Nicole Madden, medical assistant, are on the team but are not listed as prescribers.

The weight loss program runs 16 weeks and is customized to each patient. It starts with a meeting with a provider the patient selects, includes lab work at the start and end, and adds weekly 30 minute visits, prescription management and remote monitoring of a continuous glucose monitor (CGM) or Keto Mojo. What does it cost?

The weight loss program has a total cost of $2600, or $650 a month. A couples rate of $1000 a month covers two people from the same household. The program price does not include lab work or a CGM or Keto Mojo. The clinic does not say whether medication is included.

The Metabolic Health Consultation is $350 for a 60 minute consultation. The optional Metabolic Health Maintenance plan is $200 a month for two half hour visits or $100 a month for one half hour visit. It can be discontinued at any time.

Direct Primary Care with BHRT is $175 a month for ages 17 to 49, $200 a month for ages 50 to 64, and $225 a month for ages 65 and over. It carries a $200 enrollment fee for each new patient, capped at $400 for groups and families. The BHRT only membership is $75 a month, plus a one time initial consultation fee of $200.

What should you ask before you book?

Ask which prescriber would sign your prescription, Dr. Lenzkes or Leah Koskinen, PA-C, and how often you would see that person.

Ask whether the monthly fee covers the medication. If it does not, ask what the medication costs, whether it is brand name or compounded, and which pharmacy fills it.

Ask what the starting and ending labs cost, whether the discounted cash pay lab program applies to you, and what a CGM or Keto Mojo costs if you need one.

For the monthly plans, ask what follow-up visits and labs cost beyond the fee. Ask how cancellation works for the weight loss program and the direct primary care and BHRT memberships. Only the Maintenance plan states that it can be discontinued at any time, and the enrollment fee's refund terms are not stated. Ask whether any insurance is accepted, since the clinic describes its programs only as self-pay.

Who is it a good fit for?

This is a good fit if you live in California or Arizona, can pay monthly out of pocket, and want an ongoing membership with Dr. Lenzkes or Leah Koskinen, PA-C. It may not suit you if you need the weight loss medication named before you book, or if you expect labs and CGM costs to be included in the program price.
