Laguna Longevity Lab is a physician-led medical clinic at 330 Park Ave, Ste 5, in Laguna Beach, California 92651. The phone number is 949-484-6672. Dr. Cherlin Johnson is the named prescriber. Dr. Johnson is a board-certified emergency physician whose stated focus is evidence-informed longevity medicine and hormone health.

What does it cost?
The Laguna Longevity Membership is $225/month. It covers three physician visits a year, monthly check-ins, monthly prescription management, lab panel review, body composition analysis with each visit, and phone and email support. The clinic does not say whether medication is included. In its own words: "Membership is charged monthly. Cancel anytime."
Upgrades add $100/month for hormones or peptides, or $200/month for both. The clinic also says hormones and peptides are charged separately from membership fees, so ask which applies.
The $895 initial evaluation includes a 60-minute physician visit and a 60-minute follow-up visit. Other follow-up visits run $495 for 40 minutes or $250 for 20 minutes, and prescription renewals run $175 to $275. Labs, imaging, medications and specialty testing are billed separately, through insurance or self-pay. The clinic does not bill insurance for in-office services, but it will give you an invoice to send to your insurer. Smaller services include a B12 injection at $25, body composition analysis at $195, and Botox or Xeomin at $14 per unit. The clinic does not publish separate prices for GLP-1, hormone or peptide care.
What should you ask before you book?
Ask who writes and signs each prescription. The clinic names Dr. Johnson as prescriber but does not publish MD or DO credentials.
Ask whether the $225 membership covers the medication itself, or whether you pay for GLP-1 or hormone medication on top. For a GLP-1 or peptide, ask whether it is brand-name or compounded, and which pharmacy fills it. The clinic mentions pharmacy partners for its peptides but does not name them.
Ask what labs, imaging and extra visits will cost each year, and which of those fall inside the membership. Ask whether telehealth follow-ups are available where you live, since no states are listed. Ask how cancellation works, including any notice period. If you are interested in fillers or peels, ask when they will be offered, because they are marked coming soon.
Who is it a good fit for?
This is a good fit if you want a named physician to review your labs and manage a plan on a monthly membership, and you are ready to pay cash and get the medication costs in writing before you start. If you need insurance to pay for visits, this clinic does not bill insurance for in-office services.
