About Trails to Health

What is Trails to Health?

Trails to Health is a concierge and functional medicine practice in Bentonville, Arkansas. Dr. Michael Henderson, MD, is the physician, and he describes himself as board-certified in both Family Medicine and Lifestyle Medicine. Its main offering is primary care on a membership basis, though that membership is currently wait list only. The other two listed services are functional medicine with monthly visits, and a Longevity Assessment with a 2 to 3 hour in-clinic visit.

Older material on the clinic's website also lists bioidentical hormone replacement therapy, testosterone replacement therapy (TRT), weight loss programs, weight loss medication management and menopause management. Those services are not on its main services list, so ask whether they are still offered. Its blog discusses semaglutide, liraglutide, dulaglutide and tirzepatide. The clinic gives a phone number, (479)-879-9359, but no street address. It is rated 5 on Google from 29 reviews.

What does it cost?

The functional medicine initial consultation is $450 (about 60 minutes). After that, the monthly membership is listed as an average of $299 a month, customized after the consultation. Elsewhere the clinic gives a range of $399 to $499 a month, so ask which one applies. Membership has a 3-month minimum, and lab testing and specialty supplements are billed separately. Its figures put a four-month program at $1,646 and a six-month program at $2,244, both including the consultation, but another listing gives a four-month program at $2,000 total.

The Longevity Assessment is listed at $3,000, while its detailed section gives a standard rate of $2,495 and a 2025 founder rate of $2,000. Add-ons are $300 for a CGM package, $300 for nutrition tracking, and $300 to $500 for follow-up labs.

Primary care membership is $49 a month for an individual, or $25 per person for families of three or more, with a 6-month minimum. The initial adult appointment is $250, and visits are billed at $209 an hour in 15-minute increments.

It publishes no price for hormone therapy or weight loss medication, and it does not say whether medication is included in any program price. It does not say whether it takes insurance. It offers superbills and says patients report reimbursement of 20-30% on average, which varies by plan. HSA and FSA funds may apply. In its own words: "Just honest healthcare pricing designed to work alongside your insurance."

What should you ask before you book?

Ask whether the monthly fee includes any medication, and if so, which ones. Ask whether the weight loss or hormone medications are brand-name or compounded, and what they cost each month. Ask Dr. Henderson whether he writes every prescription himself.

Ask which price applies: $299 or $399 to $499 a month, $1,646 or $2,000 for four months, and $3,000 or $2,495 for the assessment. Ask whether the hormone and weight loss services are still offered, and what they cost. Ask what follow-up visits and lab work add up to, and whether telehealth is available.

Ask how cancellation works after the 3-month minimum, and whether any part of a payment is refundable. Ask for the office street address and hours. The clinic lists only a phone number and says direct communication is open Monday through Thursday, 8am to 5pm.

Who is it a good fit for?

This is a good fit if you want a physician you can reach directly, you are prepared to stay in a monthly membership for at least three months, and you are willing to get medication prices in writing before you sign. If you need primary care soon, ask about the wait list first, since that membership is wait list only.
